Job summary

Are you an experienced ANP looking for a new challenge? Chorley Together Primary Care Network are seeking an experienced Advanced Nurse Practitioner to join the team at Whittle Surgery.

Formed in May 22, Chorley Together PCN consists of six GP practices across the Chorley and Leyland locality and serves a population of approximately 38000. Whittle Surgery is located in modern purpose built premises on the A6 at Whittle-le-Woods and has a current list size of 11500 patients. You will work closely with GPs, Clinical Pharmacists, Social Prescribers, Mental Health Practitioner, First Contact Practitioner, Care Co-ordinators and practice nursing and admin teams as part of a multi-disciplinary team to deliver a holistic and patient centred approach to care.

The postholder will lead on all aspects of frailty work across the practice, including deliver of the local GPQC frailty specification. They will also lead on care home ward rounds for the practice.

The role is offered on a part time basis between 22.5hrs and 27.5hrs over 3 days per week. Exact hours and working pattern will be discussed at interview.

Main duties of the job

The role is focused on the autonomous assessment, investigation and management of patients presenting with problems within the primary care setting.

The Advanced Nurse Practitioner will be responsible for attending to patients who wish to be seen by a healthcare professional, diagnosing and treating patients presenting with minor illness, seeing and advising patients in respect of their continuing medical and nursing needs. The post holder will need to triage and prioritise the needs of patients, accordingly, making any necessary referrals for investigations in the appropriate manner.

Whittle Surgery is the largest of its member practices and has a registered list size of approximately 11500 patients. Located in modern purpose built premises on the A6 at Whittle-le-woods the practice is within each reach of the M6, M61 and M65 motorways.

  • Masters Degree in Advanced Practice from a Centre for Advancing practice accredited MSc advanced practice programme or have completed the Centre for advancing practice ePortfolio (supported) route.
