Job summary
We are looking for an Advanced Clinical Practitioner/Nurse to join our amazing team to positively contribute to the provision of high quality patient centred care.
The role is focussed on the autonomous assessment, investigation and management of patients presenting with problems within the primary care setting. You will work closely with GPs, ANP, Practice Nurses, Health Care Assistant, Clinical Pharmacists, Social Prescribers, Care Co-ordinators and practice admin teams as part of a multi-disciplinary team.
We are looking for enthusiastic, experience and caring Practitioners with excellent clinical skills to joint our team to help improve access for patients. Postholders may be required to support delivery of our enhanced access service which operates 18:30-20:00 Monday- Friday and Saturdays 09:00-17:00.
This is a fixed term contract for 12 months for maternity cover with a view to possible permanent contract
Main duties of the job
The Advanced Clinical Practitioner will be responsible for attending to patients who wish to be seen by a healthcare professional, diagnosing and treating patients presenting with minor illness, seeing and advising patients in respect of their continuing medical and nursing needs. The post holder will need to triage and prioritise the needs of patients, accordingly, making any necessary referrals for investigations in the appropriate manner.
About us
We are a 5400 patient practice and have 2 GP's, 3 nurses, an admin team of 3 people, reception team, Assistant Practice Manager and Practice Manager and a team of regular locums. We have a very loyal team of staff with vacancies only usually arising upon retirement or a new post being created. This one is arising as a maternity cover for one of our clinical team with a view to permanent contract. We are based in the lovely rural setting of Withnell, a small village on the outskirts of Chorley and receive great feedback from our patients, scoring 97% patient satisfaction in the GP survey which was the 2nd highest score in Lancashire & South Cumbria

Diagnose and manage both acute and chronic conditions, initiating, evaluating, and modifying a range of interventions which may include prescribing and reviewing medicines for therapeutic effectiveness (appropriate to patient need and in accordance with evidence-based practice and national protocols within scope of practice), therapies, lifestyle advice and care into a management plan
Work in partnership with individuals, families, and carers, using a range of assessment methods as appropriate (e.g., of history-taking; holistic assessment; identifying risk factors; mental health assessments; requesting, undertaking and/or interpreting diagnostic tests; and conducting health needs assessments).
Offer a holistic service to patients and their families, supporting people in making decisions, planning care, or seeking to make positive changes, with an emphasis on health promotion and self-care
Where the post holder is an independent prescriber: to prescribe safe, effective, and appropriate medication in line with current guidance and legislation
Work with patients in order to support compliance with and adherence to prescribed treatments, providing information and advice on prescribed or over the counter medication on medication regimens, side effects and interactions
Support and manage health needs of women presenting for family planning or sexual health consultation
Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health
Collaborate with other members of the Primary Health Care Team, accepting referrals and referring to them
Refer patients directly to other services and agencies as appropriate
Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families, and carers.
Act as a clinical role model/advocate for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies, and networks.
Deliver care according to NSF, NICE guidelines, and evidence-based care.
