Job summary

These vacancies are being advertised by Withnell Health Centre on behalf of Chorley Together Primary Care Network and its member practices.

We are seeking Advanced Nurse Practitioners to join our growing team. You will work with practices within our network to positively contribute to the provision of high-quality patient centred care. The roles are focussed on the autonomous assessment, investigation and management of patients presenting with problems within the primary care setting.You will work closely with GPs, Clinical Pharmacists, Social Prescribers, Care Co-ordinators and practice nursing and admin teams as part of a multi-disciplinary team.

We are looking for enthusiastic, experienced and caring ANPs with excellent clinical skills to join our team in supporting our practices with improving access for patients and the delivery of our care home service. Postholders will be expected to support delivery of our enhanced access service which operates 18:30-20:00 Monday- Friday and Saturdays 09:00-17:00.

Main duties of the job

The Advanced Nurse Practitioner will be responsible for attending to patients who wish to be seen by a healthcare professional, diagnosing and treating patients presenting with minor illness, seeing and advising patients in respect of their continuing medical and nursing needs. The post holder will need to triage and prioritise the needs of patients, accordingly, making any necessary referrals for investigations in the appropriate manner.

The post holder will be required to lead in the area of frailty/and or care homes to deliver a holistic and patient centred approach to care which will include home visits and/or care home ward rounds

Formed in May 22, Chorley Together are a relatively new PCN consisting of seven GP practices across the Chorley and Leyland locality.As a PCN we support the operational, strategic and financial resilience of our member practices in order to improve the health and wellbeing of our population. We work in collaboration with health, social care and third sector providers, to support people holistically from birth to the end of their lives. We strive to drive improvements in local healthcare, build strong relationships and promote integrated working and we take a leadership role in the health and social care system for the benefit of our population.

Work in partnership with individuals, families, and carers, using a range of assessment methods as appropriate (e.g., of history-taking; holistic assessment; identifying risk factors; mental health assessments; requesting, undertaking and/or interpreting diagnostic tests; and conducting health needs assessments).

Offer a holistic service to patients and their families, supporting people in making decisions, planning care, or seeking to make positive changes, with an emphasis on health promotion and self-care

Where the post holder is an independent prescriber: to prescribe safe, effective, and appropriate medication in line with current guidance and legislation

Work with patients in order to support compliance with and adherence to prescribed treatments, providing information and advice on prescribed or over the counter medication on medication regimens, side effects and interactions

Support and manage health needs of women presenting for family planning or sexual health consultation

Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health

Collaborate with other members of the Primary Health Care Team, accepting referrals and referring to them

Refer patients directly to other services and agencies as appropriate

Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families, and carers.

Act as a clinical role model/advocate for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies, and networks.

Deliver care according to NSF, NICE guidelines, and evidence-based care.

To record accurate data in the patient records to an agreed standard.
