Main duties of the job

The post-holder will provide assessment, treatment, screening, health education services and advice to patients. Working independently and autonomously alongside the General Practitioners and making decisions/recommendations as appropriate to patient care.

The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners. The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use them appropriately.

Job description

Job responsibilities

  • Manage chronic diseases and monitor therapeutic progress.
  • Diagnose and develop individual management plans for patients, in collaboration with healthcare professionals.
  • Ensure patient compliance with appropriate treatments through a holistic, patient-centred approach.
  • Identify the impact of treatments and make necessary changes within recommended guidelines for chronic disease management.
  • Recognise and address significant abnormalities.
  • Contribute to meeting Quality and Outcome Framework targets.
  • Provide wound care and management, including assessment of routine wounds, discussion of vaccine status, and referral to appropriate services.
