Main duties of the job
The post-holder will provide assessment, treatment, screening, health education services and advice to
patients. Working independently and autonomously alongside the General Practitioners and making
decisions/recommendations as appropriate to patient care.
The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing
treatments to patients both independently and in participation with nurse colleagues and general
practitioners.
The post-holder will have the required level of training and competence and will work within patient
group directions where these are available. A nurse with prescribing qualifications will use them
appropriately.

Job responsibilities
- Manage chronic diseases and monitor therapeutic progress.
- Diagnose and develop individual management plans for
patients, in collaboration with healthcare professionals.
- Ensure patient compliance with appropriate treatments
through a holistic, patient-centred approach.
- Identify the impact of treatments and make necessary changes
within recommended guidelines for chronic disease management.
- Recognise and address significant abnormalities.
- Contribute to meeting Quality and Outcome Framework targets.
- Provide wound care and management, including assessment of
routine wounds, discussion of vaccine status, and referral to appropriate
services.
- Conduct patient health checks, identify abnormalities, and
gather relevant information through lifestyle questioning.
- Develop management plans for patients with identified health
problems or risks, and provide support and education for behaviour change.
- Conduct health screenings, refer appropriately based on
identified issues, and provide monitoring and advice.
- Recognise and gather information related to risk assessment
for various social issues.
- Administer travel immunisations based on destination guidelines,
utilise approved NHS resources, and refer to specialist travel services when
necessary.
- Lead and coordinate childhood immunisation programme,
administer immunisations, and provide advice on complex immunisation issues.
- Oversee recall and DNA system for childhood immunisation
programme.
- Perform general nursing duties, including phlebotomy (if
needed), assisting in minor surgeries and coil fits, and administering
prescribed injections.
- Assist doctors in handling urgent cases as they arise.

monitor the drug cupboard and emergency drugs.
