Trinity Health Group Primary
Care Network (PCN) is a collaboration of GP practices across Wakefield, serving
a population of approximately 55,000 patients. As part of our commitment to
improving the quality of patient care, reducing health inequalities, and
supporting the NHS Long Term Plan, we are expanding our team to further develop
integrated, person-centred services.
We are seeking a Registered
Mental Health Nurse to join our established and supportive Mental Health
team. You will provide high-quality mental health nursing care, offering
advice, support, and signposting to patients experiencing poor mental health.
Working collaboratively with GPs and the wider primary care team, you will play
a vital role in strengthening community resilience and delivering coordinated
care across our four member practices.
Trinity PCN values the
essential role of Mental Health Nurses as part of a wider multi-disciplinary
team that includes Care Coordinators, Health and Wellbeing Coaches, Social
Prescribing Link Workers, Paramedics, Pharmacists, Pharmacy Technicians, and others
across the voluntary, community, and statutory sectors, including the ICB.
Main duties of the job
Mental Health
Nurses in primary care complement the role of the GP and other Health
Professionals as integral members of the multidisciplinary team. To make the
best use of clinical skills within the practice in order to promote well-being
and maximise health gain for patients. Provide a safe, high quality and
accessible treatment room service to the practice population. The successful
candidate with be working alongside our existing Mental Health Nurse
Practitioner to provide a much needed and well-utilised service for our
locality.
About us
This role is hosted by Conexus
Healthcare, the GP Confederation for Wakefield District, which supports
local PCNs by employing staff and providing operational and clinical support.
While you will be employed by Conexus, your day-to-day work will be PCN-based
under the direction of the PCN Business Manager and Clinical Directors.
