Are you an enthusiastic and forwards thinking qualified Clinical Pharmacist?
If so this is an
exciting opportunity for you to join our existing pharmacy team, working with general practices across our PCN footprint,
with support from our existing clinical teams to deliver excellent patient care. You will be part
of our growing multi-disciplinary additional roles team, who all work together
to support our GP practices and their patients.The roles
currently include Care Coordinators, Clinical Pharmacists, Pharmacy
Technicians, Paramedics, Health and Wellbeing Coaches, Social Prescribing Link
Worker, Outreach Nurses and Mental Health Practitioner.
Main duties of the job
The post holder will work within their clinical competencies as part of a
multi-disciplinary team to provide expertise in clinical medicines management,
provide face-to-face structured medication reviews, manage long term
conditions, management of medicines on transfer of care and systems for safer
prescribing, manage repeat prescription authorisations and reauthorisation,
acute prescription request, while addressing both the public health and social
care needs of patients in the GP practice(s) that make up the PCN.
The base for this role will be at Henry Moore Clinic, but working with all six practices within the PCN.

Wakefield North Primary Care Network (PCN) are developing our services to
support our six general practices and patients in our PCN across the Eastern
side of Wakefield District, serving a population of approximately 48,700. Our practices
are;

- Tieve Tara Medical Centre
- Station Lane Medical Centre
- St Thomas Road Surgery
- Newland Surgery.
