Job summary

Working as part of the PCNs multi-disciplinary
team, the post holder will support our GP practices patients take steps to
improve the way they manage their health conditions around what matters to them. Using
coaching skills, patients will be supported to address existing issues and
promote lifestyle medicine to develop skills, knowledge and confidence to
manage and improve their health outcomes, wellbeing and quality of life.
Main duties of the job
As an integral part of the PCN, the postholder will use coaching techniques to provide one-to-one and group guidance, advice and coaching to support patients, identifying their needs and setting goals to achieve their personalised health and care plan objectives.The role is varied and allows the Health and Wellbeing Coach to meet with patients in their GP practice either face-to-face, on the telephone or via video calling.The role also involves working in the community at pop-up sessions to offer self-management education on healthy lifestyles.
The role has access to ongoing supervision and support to allow the Health and Wellbeing Coach to develop their skills and experience.
The base for this role will be Fryston Road Surgery, but working with all six practices within the PCN

- Must be willing to attend training with a non-clinical Supported Self-Management (SSM) health coaching skills programme (minimum four days) by a Personalised Care Institute (PCI) accredited trainer or organisation prior to taking referrals.
Desirable
- Coaching/counselling qualification/ experience or other relevant qualification/experience involving reflective listening skills. Relevant training and experience in non-clinical Supported Self-Management (SSM) Health Coaching through a PCI-accredited organisation
Experience

- Experience of using coaching approaches/frameworks and models, or other helping strategies, eg Motivational Interviewing.
- Experience of working with patients on nutrition and health.
