Job summary

Trinity Health Group PCN is looking for a Clinical Pharmacist to join our current team of 3 Clinical Pharmacist and 3 Pharmacy Technicians. The post holder will work alongside the existing team to support meeting the needs of our patients.

Main duties of the job

The post holder will be undertaking structured medication reviews, improving medicine optimisation and safety, supporting care homes, as well as running practice clinics, as set out in the PCN DES. Part of this role will also include working within our Pharmacy Hub alongside the Pharmacy Team to provide a safe and effective Pharmacy Service. To provide professional specialist medicines advice to GPs, Nurse Practitioners, Practice Nurses, Community Pharmacists, Community Nurses, and other health professionals to promote the safe, effective, and high quality, cost-effective use of medicines in all prescribing decisions for the benefit of the Practice's patients. You will also be actioning acute prescription requests and conducting medicines reconciliation.

About us

Trinity Health Group is a PCN made up of four practices on the west of the district (Trinity Medical Centre, Maybush Medical Centre, Crofton and Sharleston Medical Practice and Warrengate Medical Centre). The PCN team currently consists of Clinical Pharmacists, Pharmacy Technicians, Paramedics, Mental Health Practitioners, FCPs, Care Coordinators, Health & Wellbeing Coaches, GPAs and Social Prescribing Link Workers. Qualifications

Essential

  • Master's degree in Pharmacy or equivalent
  • Registration with the General Pharmaceutical Council (GPhC)
  • Independent Prescriber - or willing to work towards
  • Completion of the Primary Care Pharmacy Education (CPPE) pathway - or willing to work towards

Desirable

  • Postgraduate Diploma in Clinical Pharmacy (or equivalent)

Experience

Essential

  • Evidence of working autonomously or as part of a team
  • Medicines Management
  • Experience of communicating with other healthcare professionals and the general public
  • Experience of prioritising workload.
  • Experience of using own initiative to solve problems.
  • Evidence of personal leadership to improve the quality of care.

  • Policy / procedure development General Practice repeat prescribing systems.
