Job summary

Trinity Health Group PCN is looking for a Clinical Services Manager to support our developing PCN Management Team. The postholder will work closely with the existing PCN Management Team and Clinical Directors.

Main duties of the job

Trinity Health Group PCN has employed a large number of staff over the last 3 years, a significant proportion of these staff are clinical. The Clinical Service Manager role will support the PCN Business Manager to drive the development of a sustainable PCN, specifically supporting the clinical development of the PCN, and identifying service development and redesign opportunities to deliver efficiencies in care delivery and improvements in patient experience for the PCN population.

Trinity Health Group is a PCN made up of four practices on the west of the district (Trinity Medical Centre, Maybush Medical Centre, Crofton and Sharleston Medical Practice and Warrengate Medical Centre). Job responsibilities

Role Specific Responsibilites:

The post will evolve as we develop our services but the postholder will be required to undertake the following duties:

Clinical Management

  • Ensure that patient safety is at the forefront of all service developments and delivery, including writing and maintaining protocols for clinical and non-clinical staff across the PCN.
  • Develop quality monitoring and support mechanisms for any PCN staff and relevant subcontractors. This will include completing clinical audits across all ARRS roles and feeding back to relevant individuals and CDs
  • Working with the PCN Business Manager to create and develop ideas providing suggestions as to how we can deliver the DES specification to ensure the success of the network
  • Understand and identify risk and implement necessary mitigating actions and report significant risks and proposed actions through DATIX and other local reporting systemsResponsible for the day to day running of the clinical services within Trinity Health Group PCN. This includes the operational management and leadership of clinical staff, resources, support services, subcontractors and achievements of KPIs and outcomes
  • Support the delivery of clinical services in partnership with the PCN Business Manager/ Clinical Directors, including being the clinical lead on relevant PCN projects ensuring successful implementation, monitoring and evaluation against expected outcomes and in line with timescales
  • Support the clinical teams to deliver care that helps improve health outcomes for individuals/communities
  • Work with the Business Manager and service leads to identify service development and redesign opportunities that deliver efficiencies in care and/ or improvements in patient experience.
  • Communicate effectively within the team to ensure others can carry out their work efficiently and in compliance with professional standards and legislation
  • Maintain and develop own clinical and professional competence to deliver effective care for patients, and effective leadership for the team.

Resource Management

  • Assist the Business Manager to manage resources in order to formulate, mobilise and ensure the ongoing implementation of plans to any agreed specification across all roles within the PCN
  • Support and encourage learning to continually improve network services and patient outcomes
  • Work in collaboration with the PCN Business Manager to develop the PCN workforce which will involve assisting with recruitment, deployment, line management, mentoring and appraisal. Facilitate the development of induction programmes for new staff within the PCN Clinical services
  • Identify and deal quickly, fairly, and consistently with poor performance in line with HR policy
  • Provide operational support and leadership to team leaders within the PCN to support clinical governance and implementation of clinical services
  • Work with the PCN Business Manager to oversee any sub-contracting arrangements, negotiating effective use of resources, agreeing service level agreements or contracts and monitoring delivery on behalf of the respective networks
  • Responsible for leading, motivating and effectively managing Team Leaders, subcontractors, and relevant team members within the PCN by providing excellent clinical and managerial leadership. Supporting the training and development of PCN clinical staff
  • Support the PCN Business Manager in identifying potential sources of funding and submitting bids as appropriate to support the development of both the staff and the PCN
  • Ensure all clinical staff in Additional Roles are supported through supervision
  • Utilise routine data reports to facilitate adequate monitoring of PCN and Team performance, using relevant data to inform 1:1 staff meetings
  • In partnership with the PCN Business Manager set performance targets, ensuring these are reviewed regularly and any necessary actions are taken to address areas for improvement.
  • Collate feedback / analyse data on behalf of the network to report to the ICS/Wakefield Place
  • Undertake analysis of performance data and capacity and demand data to identify areas for service redesign and collaborative working.

Relationships

  • The post holder will have constructive relationships with internal and external stakeholders, including but not limited to the following:
  1. Clinical Directors & Nurse Lead for the PCN network
  2. Practices Managers, support staff & lead GPs at member practices
  3. Primary care team at ICS and Wakefield Place
  4. Wider Integrated Care Partnership in Wakefield
  5. Other leadership teams in Wakefield PCNs
  • Act as a first point of contact for Practice Managers regarding the delivery of relevant Network projects/schemes
  • Attend meetings as appropriate to the role including PCN Operational Meetings, PCN Board meetings, ICS/ICB meetings and Clinical Director meetings. Must be available to attend District wide PCN meetings, Business Manager Meetings and Practice Manager Meetings as required in the absence of the PCN Business Manager
  • Ensure effective communication is maintained within the Network
  • Listen to and respond to the needs of people with lived experience to shape and develop services
  • Maintain an excellent level of understanding and awareness of the Primary Care landscape on a District and National level
