Job summary
Join our
team as a Mental Health Nurse to provide support, advice, and signposting to
patients with poor Mental Health. You will be a registered Mental Health Nurse,
with the NMC.
We will provide a supportive learning
environment, helping you to set out and revise specific educational goals and
ensure you receive appropriate clinical supervision. You will be supported and
encouraged to attend CPD enabling you to grow and develop within the role.
Mental Health Nurses in primary care complement the role of the GP and other Health Professionals as integral members of the multidisciplinary team. To make the best use of clinical skills within the practice to promote well-being and maximise health gain for patients. Provide a safe, high quality and accessible treatment room service to the practice population. The successful candidate with be working alongside our existing Mental Health Nurse Practitioner to provide a much-needed and well-utilised service for our locality.
Having achieved the independent prescribing qualification is desirable but not essential.
We recognise and value the importance
of Mental Health Nurses as part of our wider clinical teams which comprises a wide range of colleagues particularly Care
Co-ordinators, Health and Wellbeing Coaches, Social Prescribing Link Workers, and Occupational Therapists. Working alongside the wider PCN team, our member practices, and the Voluntary and Community Sector, as well as statutory service
providers.
Main duties of the job
Main Duties
See patients with low-risk mental health problems who have been referred by:
GP's or triage nurses working for a Practice within the network, and to:
v Monitor medications and progress
v Provide brief intervention
v Provide information.
v Refer on to other services
v Plan follow up appointments
Assesses patients needs and develop, implement and evaluate intervention
plans as required.
- Provides health education and advice on health promotion to patients.
- Responds appropriately to crises, where necessary seeking advice and
guidance from other relevant professionals.
Administration and record-keeping:
Ordering of Health Promotion Literature
Participation in administrative systems in the practice
At all times there will be a need to maintain accurate records
Attend and participate in any practice meetings when required.
Other duties may be required to meet the responsibilities of the post and the requirements of the organisation.
