Job summary

We are West Wakefield Primary Care Network and we are developing a wide range of services to support general practices and improve outcomes for patients across the West of Wakefield.

We would like you to join our excellent and supportive team as a Pharmacy Technician to provide safe, quality, and cost-effective pharmacy services in primary care working with a wide range of colleagues within the Primary Care Network (e.g. Care Co-ordinators, Health and Wellbeing Coaches, Social Prescribing Link Workers, Nurse Associate, Advanced Nurse Practitioners, Occupational Therapists); alongside the wider team including the Practices, Community Pharmacy, the Voluntary and Community Sector, as well as statutory service providers including the CCG.

Our aim is to create a diverse team that support one another, have opportunities to share learning and engage in professional development, while being integrated into the practices. You will work closely with the existing clinical teams to deliver excellent patient care working innovatively and collaboratively with colleagues.

In this role you have the potential to help manage general practice workload and significantly improve quality of care and safety for patients. Feedback from patients for the work the team is really positive and the service is very much appreciated

You will be employed by Conexus Healthcare Limited (the confederation of General Practice in Wakefield).

Main duties of the job

Working as part of a multi-disciplinary team in a role that will mix service development work with a patient-facing role. You will need to be committed to improving health and putting patients first. Your ability to build effective relationships with patients, GPs, nurses, community pharmacists, hospital and practice staff will enable you to succeed in this role, alongside your excellent communication skills and ability to work effectively as part of a team.

The role may involve occasional travel to 5 local practices, local care homes, and occasionally further afield. The main duties of the role include:

  • Undertakingmedicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly.
  • Managing acute prescription requests
  • Undertaking cost-effective drug switchesand de-prescribing in line with clinical guidelines and best practice
  • Undertaking discharge summary and clinic letter reconciliation
  • Providing advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness, and promote the uptake of electronic Repeat Dispensing (eRD) and electronic prescription service (EPS).
  • To participate in medication audits.

Job responsibilities

Your main responsibilities will be to support the Primary Care Network to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes. The role will also include training and development of administrators to support improved co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes.

The main responsibilities of the job will be:

  • To undertake medicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly.
  • Manage acute prescription requests
  • Undertake cost-effective drug switches and de-prescribing in line with clinical guidelines and best practice
  • Undertake discharge summary and clinic letter reconciliation

  • To support the achievement of the practices prescribing Quality and Outcomes Framework targets

  • Working with the Practices to contribute to the review and development of repeat prescribing protocols / Standard Operating Procedures to improve the efficiency and effectiveness of repeat prescribing systems.
  • To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness, and promote the uptake of electronic Repeat Dispensing (eRD) and electronic prescription service (EPS).
  • To participate in medication audits.
  • To assist in the appropriate monitoring and management of prescribing budgets and to liaise with the medicines management team at the CCG.
  • To prepare evidence-based resources and information to support the implementation of rational cost-effective prescribing.

Responsibilities for Care Homes

  • Providing safe, cost-effective clinical and technical pharmacy services to nursing and residential care home residents and older people within care communities.
  • Proactively working as part of a multi-disciplinary team that includes community nursing teams, GPs, nurses, care home staff, pharmacists, pharmacy technicians, non-medical prescribers, dietitians and support staff.
  • Accurately recording and reporting activity and outcome data as required to support quality improvement initiatives.
  • Working as part of a multi-disciplinary team to produce individually-tailored medicines management action plans, triage patients for pharmacy technician or pharmacist medication review
  • Providing medicines awareness training sessions for carers and inhaler training.
  • Building relationships within the care home, GP care home leads, primary care network pharmacist and other pharmacy technicians.
  • Providing prescribing support to nominated general practice(s) within their ICS including advice regarding the transfer of care procedures for care home residents.
  • Providing education and training to colleagues including the safe and appropriate ordering, storage and administration of medicines within a care home environment.
