We are West Wakefield Primary Care Network and we are developing a wide range of services to support general practices and improve outcomes for patients across the West of Wakefield.
We would like you to join our excellent and supportive team as a Pharmacy Technician to provide safe, quality, and cost-effective pharmacy services in primary care working with a wide range of colleagues within the Primary Care Network (e.g. Care Co-ordinators, Health and Wellbeing Coaches, Social Prescribing Link Workers, Nurse Associate, Advanced Nurse Practitioners, Occupational Therapists); alongside the wider team including the Practices, Community Pharmacy, the Voluntary and Community Sector, as well as statutory service providers including the CCG.
Our aim is to create a diverse team that support one
another, have opportunities to share learning and engage in professional
development, while being integrated into the practices. You will work closely
with the existing clinical teams to deliver excellent patient care working
innovatively and collaboratively with colleagues.
In this role you have the potential to help manage general practice workload and significantly improve quality of care and safety for patients. Feedback from patients for the work the team is really positive and the service is very much appreciated
You will be employed by Conexus Healthcare Limited (the confederation of General Practice in Wakefield).
Main duties of the job
Working as part of a multi-disciplinary team in a role that will mix service development work with a patient-facing role. You will need to be committed to improving health and putting patients first. Your ability to build effective relationships with patients, GPs, nurses, community pharmacists, hospital and practice staff will enable you to succeed in this role, alongside your excellent communication skills and ability to work effectively as part of a team.
The role may involve occasional travel to 5 local practices, local care homes, and occasionally further afield. The main duties of the role include:
- Undertakingmedicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly.
- Managing acute prescription requests
- Undertaking cost-effective drug switchesand de-prescribing in line with clinical guidelines and best practice
- Undertaking discharge summary and clinic letter reconciliation
- Providing advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness, and promote the uptake of electronic Repeat Dispensing (eRD) and electronic prescription service (EPS).
- To participate in medication audits.
