Job summary

To support the Primary Care Network team in the delivery of nursing services, working as part of the PCN multi-disciplinary team to complement the existing workforce. This role will provide nursing support to the PCN Nurse (Clinical Care Co-ordiantor), Practice Nurses and General Practitioners to ensure provision of holistic and person-centred care of the full range of nursing services to the entitled patient population.

This role will assist clinical staff across the PCN in the provision of treatment, preventative care and health promotion. You will work as a member of the Nursing team, under the guidance of a Registered General Nurse, and must work at all times within your own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC).

You will be responsible for the care delivered, focusing on supporting patients to be healthy, monitoring of long-term conditions, health prevention and education advice and screening activities. You will work collaboratively to meet the needs of patients, ensuring the highest standards of care and supporting the delivery of policy and procedures.

Main duties of the job

Main duties of the role include, but are not limited to:

  • Enable patients, families and carers to develop their knowledge and understanding about their health and well-being and be as self-managing as possible.
  • To assist and perform routine nursing tasks related to patient care
  • To assist and perform routine treatment room duties
  • Undertake simple dressing procedures
  • To support patients to improve and maintain their mental, physical, behavioural health and wellbeing
  • To support chronic disease management
  • Provide healthy lifestyle advice regarding diet/exercise/smoking
  • Advise patients on general healthcare and promote self-management where appropriate
  • Support immunisation programmes.
  • To provide support and supervision to staff in training
  • The collection of clinical waste
  • Ensure adequate infection control procedures
  • To maintain accurate patient records
  • Participate in meetings and projects within the PCN.
  • Assessment and planning of delegated care.
  • Administering of Medication, where necessary.
  • Be able to perform CPR according to current UK guidelines.
  • Communicate effectively with other members of the primary health care team and patients.
  • Empathise with, and support people to improve and maintain their mental and physical health and wellbeing.
  • Act in the best interests of service users, carers, families and communities cared for.
  • Use knowledge and experience to make evidence-based decisions and solve problems.
  • Participate in Quality Improvement activities.

About us

Wakefield North Primary Care Network (PCN) are developing our services to support our six general practices and patients in our PCN across the East side of the Wakefield District. We are developing a team to visit our house bound patients and would like you to join us as a Nursing Associate. The role will also include time working within our general practices, under the leadership of our PCN Nurse (Clinical Care Co-ordinator) and the Practice Nurses for all aspects of nursing care. You will have completed your Nursing Associate training and be registered with the NMC.

Our PCN is committed to improving the care provided to our patients, reducing health inequalities and raising the standards and quality of our general practices.

You will be employed by Conexus Healthcare Limited (the confederation of General Practice in Wakefield). They have four areas of work.

  1. Delivery of high-quality public facing Health and Wellbeing services across the Wakefield District
  2. Delivery of Training, Development and Consultancy support across the United Kingdom
  3. Supporting Our Practices and Primary Care Networks (PCNs)
  4. Providing a voice and influence for General Practice in the Health and Care System
