Job summary
We are West Wakefield Primary Care Network and we are developing a wide range of services to support general practices and patients in our PCN across the Wakefield district.
We would like you to come and join our Nursing Associate team providing a mix of practice (the majority of the time) and community based services to patients including housebound, care homes, and CVD outreach. You will join a supportive team, have the opportunity to work independently under the leadership of registered nurses, and have lots of opportunities to undertake training and develop your role so that you can provide a comprehensive service with a broad range of skills. Ideally you will have completed your Nursing Associate training and be registered with the NMC.
We will provide a supportive learning environment, especially structured in the first preceptorship year. We will help you set out and revise specific educational goals and ensure you receive appropriate clinical supervision, you will receive regular appraisals. You will be supported to attend agreed CPD.
We are committed to improving the care provided to patients, reducing health inequalities and raising the quality and standards of GP practice. This role will enable you to deliver person-centred co-ordinated care, and support implementation of the NHS long-term plan.
You will be employed by Conexus Healthcare Limited the local GP confederation.
Main duties of the job
We will provide a supportive learning
environment, especially structured in the first preceptorship year. We will

appropriate clinical supervision. You will be supported and encouraged to
attend CPD enabling you to grow and develop within the role.

Co-ordinators, Health and Wellbeing Coaches, Social Prescribing Link Workers, Advanced
Practitioners, Occupational Therapists, Pharmacists and Pharmacy Technicians;
alongside the wider team including the Practices, Community Pharmacy, the
Voluntary and Community Sector, as well as statutory service providers
including the CCG.
The role will assist the registered staff:
To compliment the role of the GP and other Health Professionals as an integral part and key member of the care team.
To make the best use of clinical skills within the practice in order to promote well-being and maximise health gain for patients.
To provide a safe, high quality and accessible treatment room service to the practice population.

Job responsibilities
The post will evolve as we develop our services but you will be required to undertake the following duties in practice, patient's homes, care homes, and in the community:
- To undertake all duties in line with the training
and instruction provided by the Practice Nurse and General Practitioners in
Practice
- Assistance at clinics ensuring patient care by
chaperoning and assisting patients to dress and undress.
- Conduct weight and height measurements, test and
record blood pressure and blood sugar levels, pulse rate and temperature.
- Treatment room duties in line with competency
e.g. Suture/clip removal, Basic wound care, ECG. Phlebotomy, Urinalysis,
Venepuncture and the taking and preparing of pathology samples.
- Undertake ECG and Spirometry testing.
- Undertake simple dressing procedures following
practice guidelines and using aseptic technique
- Assist with the provision of health checks
- Support immunisation programmes
- Provide healthy lifestyle advice regarding diet/exercise/smoking
- Assist in minor surgery procedures, implant
insertions/removals, coils using aseptic technique
- To ensure adequate clinical stock levels and
maintain an effective stock management system.
- Ensure adequate infection control procedures in
treatment and consulting rooms.
- To maintain accurate patient records and enter
onto the computer using agreed Read Codes
