Job summary
The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients.
To work as part of an MDT within the PCN and practices to provide patient-focused care responsive to health need demonstrating strong communication at all levels across the organisation.
To work in partnership with the practice and the wider PCN team in developing the role further.
Maintain accurate and contemporaneous health records within SystmOne appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient healthcare.
Communicate effectively with patients and their family and carers where appropriate communicating sensitive and complex information regarding their physical health needs, results, findings, and treatment choices.
To provide first point of contact direct access for patients of all ages with undifferentiated and undiagnosed problems seeking a primary health care service including, telephone triage, face to face consultations, and home visits.
To assess, triage and manage patients within a variety of settings including home visits with undifferentiated and undiagnosed health care needs including investigating, treating, and referring patients as appropriate.
To order investigations, follow-up results and arrange appropriate management including seeking advice where appropriate.
Main duties of the job
The successful candidate will have a Post Graduate Degree Qualification in Paramedic Science, Clinical Practice, or equivalent qualification, with evidence of academic study and practical experience of minor/major illnesses and injuries that are typically encountered in a primary care setting.
You will need to be registered with HCPC. It is a requirement for the candidate to have attained a post graduate qualification in Prescribing or be working towards qualification.
To provide a first point of contact for patients with undifferentiated and undiagnosed health needs at the most appropriate time and place.
Provide specialist advice and support to patients, their families and other healthcare professionals following assessment, diagnosis and through treatment.
Provides and receives highly complex information; develops communication skills/present complex information.
To provide advanced assessment skills in history taking and clinical examination to formulate a diagnosis and initiate an appropriate plan of care, including referral to other health and social care professionals, as necessary.
Instigate and interpret diagnostic investigations as appropriate.
Provide patient and carer information, health education, and health promotion as appropriate.

An exciting opportunity has arisen for the employment of an ACP/ ANP to join Trinity Health Group PCN as part of Conexus Healthcare CIC to deliver additional same day capacity into the PCN practices.
Primary Care Networks (PCN) are groups of GP practices working closely together - along with other healthcare staff and organisations - providing integrated services to the local population. Trinity Health Group situated in Wakefield providing healthcare services to around 60,000 patients. The member Practices (Crofton and Sharlston Medical Practice, Maybush Medical Centre, Trinity Medical Centre, Warrengate Medical Centre) use a collaborative approach including the use of additional roles to strengthen communities and reduce inequalities in health. The additional roles are key to the PCNs achieving the IIF targets from the Network Contract Directed Enhanced Service.
Conexus Healthcare Ltd. is a GP Confederation in Wakefield. In addition to national training, and local clinical services, Conexus provide support to local PCNs by hostingstaff and supplying services such as HR and finance support. The successful candidate will be an employee of Conexus Healthcare Ltd. However, their work will be practice-based under the direction of the clinical director(s) and the host practices.
