Job summary
We are Wakefield North Primary Care Network (PCN) and are a
group of six GP practices based in the Eastern side of the Wakefield District
(Riverside Medical Centre, Henry Moore Surgery, Tieve Tara Medical Centre,
Station Lane Medical Centre, St Thomas Road Surgery and Newland Surgery). We are looking to expand our team of Health and Wellbeing
Coaches and would like you to join our PCNs multi-disciplinary team which
currently includes, a Social Prescriber Link Worker, Health and Wellbeing
Coaches, Care Co-ordinators, Paramedics, Pharmacists, Pharmacy Technicians, and
First Contact Physiotherapists, based in our GP Practices working alongside
practice staff and the voluntary and community sector.
As our Health and
Wellbeing Coaches collaborate with peers from two neighbouring Primary Care
Networks, you will also work with patients from other GP practices in our
locality and benefit from the support and closer working relationship from a
wider team of coaches that work with these GP practices.
We are looking to hold interviews face-to-face on Tuesday 24th October.
Main duties of the job
Work alongside the multi-disciplinary General
Practices teams within the Primary Care Network to effectively plan and implement
a high-quality Health and Wellbeing Coaching service, as part of a broader
primary care team.
Responsible for delivering group and one-to-one
sessions on areas such as healthy eating, physical activity along with general health
and wellbeing to support personalised care plans.
Play a critical role in engaging patients,
using health coaching techniques to support them take an active role in their
health.
Work autonomously, supporting patients and
their carers to manage their physical and mental health and wellbeing, build
community resilience and make informed decisions and choices when their health
changes.
Deliver a universal and proactive offer of
support to people with long-term physical and mental health conditions, building
knowledge, skills and confidence to live well with their health condition.

and deliver better patient care and care for the health needs of their

Providing a voice and influence for General Practice in
the Health and Care System: We provide a forum to develop a unified voice for General Practice and
support general practices to cooperate and work collaboratively with the wider
health and care sector partners in the district.

Essential
- Educated to GCSE A-C, or equivalent
- Technical, vocational or part-professional qualification at vocational degree level or equivalent experience (NVQ 4).
- Must be enrolled in or prepared to be enrolled in, or undertaking/qualified from an appropriate health and coaching training covering topics outlined in the NHS England and NHS Improvement Implementation and Quality Summary Guide, with the training delivered by a training organisation listed by the Personalised Care Institute.
Desirable
- NVQ Level 3 or equivalent qualification in coaching, mentoring, information advice and guidance or similar
- Qualification in Nutrition

- Ability to actively listen, empathise with people from all backgrounds and communities, respecting lifestyles and diversity and provide person-centred support in a non-judgemental way
