Job summary

We are West Wakefield Primary Care Network, a forward-thinking and innovative network, developing a wide range of services to support general practices and improve outcomes for patients across the West of Wakefield.

We are passionate about developing and delivering excellent quality local services to meet the needs of our patients and reduce health inequalities. We are looking to expand our team by recruiting A Health & Wellbeing Coach Team Leader to work alongside our practices and PCN staff. Our PCN staff team includes clinical pharmacists, pharmacy technicians, social prescribers, health and wellbeing coaches, care co-ordinators, nurse associates, occupational therapists, mental health nurses, and paramedics.

Health and Wellbeing Coaches work as a key part of the primary care network (PCN) multi-disciplinary team. As a health and wellbeing coach, you consider the whole person in addressing existing issues, encouraging the proactive prevention of new illnesses, and promoting lifestyle medicine approaches. Health coaching revolves around using coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.Our team work flexibly to meet the needs of our patients but also allowing them some flexibility to suit their needs.

Main duties of the job

The post will evolve as we develop our services but the postholder will be required to undertake the following duties;

1.Take referrals from the wider practice and PCN Teams.

2. Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement and achieve their personalised health and wellbeing objectives. This will include sitting in consultation with a patient and providing them with advice, guidance, and a management plan, personalised to their individual needs. It could include dietetics and healthy eating, lifestyle medicine and getting active, safe activities, and personal coaching and motivation.

3. Ensure all interventions and coaching are designed to empower patients to be active participants in their healthcare, empower them to manage their health and wellbeing, and live independently.

4. Support and deliver group consultations, with clinical and non-clinical colleagues, implementing new approaches to health across primary care and supporting individuals to access peer support.

5.It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the coach role.

3.Supporting Our Practices and Primary Care Networks (PCNs): We support general practices and PCNs across the Wakefield district to be more resilient, sustainable and deliver better patient care.

4.Providing a voice and influence for General Practice in the Health and Care System: We provide a forum to develop a unified voice for General Practice and support general practices to cooperate and work collaboratively.

Built within primary care, for primary care, Conexus Healthcare CIC works with local GPs and their practices to champion investment in and deliver effective primary care at scale. By connecting in this way, we are stronger, more resilient and can care effectively for over 370,000 local people together.

Key Tasks

Line Management

Conduct regular (6 weekly) one to ones/schedule performance reviews in line with PCN and Conexus Policies.

Develop Personal Development Plans, monitor progress and make amendments in collaboration with your direct reports.

Suggest, plan, and organise team building and training activities.

Ensure compliance with Mandatory training for your team.

Pre-empt, and effectively manage conflicts within the team.

Approve and Manage Absence requests for your team, ensuring that the service is appropriately staffed, and task boxes are monitored.

Referrals

Promoting health coaching, its role in self-management, and the wider determinants of health.

Build relationships with key staff in GP practices within the local Primary Care Network (PCN), attend relevant meetings, become part of the wider network team, and give information and feedback on health and wellbeing coaching.

Provide personalised support

Meet people on a one-to-one basis, making home visits where appropriate within organisations policies and procedures. Give people time to tell their stories and focus on what matters to me. Build trust with the person, provide non-judgmental support, respecting diversity and lifestyle choices. Work from a strength-based approach focusing on a persons assets.

Be a friendly source of information about wellbeing and prevention approaches.

Help people identify the wider issues that impact their health and wellbeing, such as debt, poor housing, being unemployed, loneliness, and caring responsibilities.

Work with individuals to co-produce a simple personalised support plan based on the persons priorities, interests, values, and motivations including what they can expect from the groups, activities, and services they are being connected to and what the person can do for themselves to improve their health and wellbeing.

Where appropriate, physically introduce people to community groups, activities, and statutory services, ensuring they are comfortable. Follow up to ensure they are happy, able to engage, included, and receiving good support.

Where people may be eligible for a personal health budget, help them to explore this option as a way of providing funded, personalised support to be independent, including helping people to gain skills for meaningful employment, where appropriate.

Support community groups and VCSE organisations to receive referrals

Forge strong links with local VCSE organisations, communities, and groups, utilising their networks and building on what is already available to create a map or menu of community groups and assets. Use these opportunities to promote micro-commissioning or small grants if available.

Develop supportive relationships with local VCSE organisations, community groups, and statutory services, to make timely, appropriate, and supported referrals for the person being introduced.

Ensure that local community groups and VCSE organisations being referred to have basic procedures in place for ensuring that vulnerable individuals are safe and, where there are safeguarding concerns, work with all partners to deal appropriately with issues.
