An Exciting opportunity has arisen for the employment of an ACP to join the team, hosted within the Five Towns PCN and supporting the Health Inclusion agenda across the Wakefield District.

Main duties of the job
We are looking for a motivated self-starter to join us, working with Health Inclusion cohorts across the district. There is a newly established Health Inclusion service in the district, the post holder will work alongside the service, our practices and PCNs to support primary care outreach. Working with a range of different people across different locations, your role will be to provide patient-focussed care, maintain strong connections between General Practice, PCNs and the community, and help us guide the development of people-centred care both now and for the future.
In return, you will receive the full support of Conexus Healthcare CIC as your main employer, with daily management coming from the Clinical Director and host PCN who you will be working with every day. You will have an opportunity to frame the role and reduce health inequalities.

Conexus Healthcare is the GP federation (a group of GP practices) for all the practices across the Wakefield District in West Yorkshire. Collectively, we provide a platform for the voice of general practice across the district. We work collaboratively with and on behalf of primary care networks and practices to improve health, well-being and patient experience in the Wakefield District and beyond.

- RGN / RN with Current NMC Registration
- MSc or BSc, Nurse Practitioner Degree
- MSc Advanced Clinical Practice (which meets the requirements of Health Education England regional / national framework)
- Evidence of continuing professional development
Desirable
- Mentor Qualification
- Chronic Disease Management certificates e.g. COPD/asthma/diabetes/IHD/women's health/sexual health
Experience
Essential
- Substantial post-registration experience
- practical experience of clinical procedures
- experience planning and managing clinics
- previous experience of working with SystmOne
