Job summary

We are West Wakefield Primary Care Network, a forward-thinking and innovative network, developing a wide range of services to support general practices and improve outcomes for patients across the West of Wakefield.

We are passionate about developing and delivering excellent quality local services to meet the needs of our patients and reducing health inequalities. We are looking to expand our team by recruiting a team of Advanced Clinical Practitioners who come from either a Nursing, Paramedic or Pharmacist background to work alongside our practices and PCN staff. Our PCN staff team includes clinical pharmacists, pharmacy technicians, social prescribers, health and wellbeing coaches, care co-ordinators, nurse associates, occupational therapists, mental health nurses, and paramedics.

Your role as an ACP is key to the development of our PCN. You will provide the first point of contact and direct access for patients of all ages with undifferentiated and undiagnosed problems seeking a primary health care service including on-call, telephone triage, face-to-face consultations, and home visits. You will need to be able to work autonomously, however, collaboration within practices and the wider PCN team is a key part of the role. Your aim will be to provide patient-focused care responsive to health need in a mix of practice-based clinics, home visits, and care homes.

We are looking to recruit 1 to 2 roles to work across all of the Practices listed in collaboration with our team.

Main duties of the job

Working as part of a multi-disciplinary team in a role that will mix service development work with a patient-facing role. You will need to be committed to improving health and putting patients first. Your ability to build effective relationships alongside your excellent communication skills and ability to work effectively as part of a team will enable you to succeed in this role.

The role may involve travel to the 5 practices, local care homes, and occasionally further afield.

Main duties:

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Implement and participate in vaccination and immunisation programmes for both adults and children

Participate in multidisciplinary clinics, ward rounds, patient reviews and MDT meetings.

Provide a seamless, high quality service from referral through to assessment, diagnosis, treatment and review, referring to other specialists as required. Provide guidance to staff, patients and their families and carers on pathway navigation

Participate in multidisciplinary clinics, participate in ward rounds, patient reviews and multidisciplinary team meetings

Undertake a variety of clinical skills and provide treatment/ advice as per speciality and scope of practice. Using agreed protocols of clinical practice and professional guidelines. This service may be provided within in primary, community, care home, or home care settings

Within scope of practice and clinical competence request and / or undertake diagnostic procedures and clinical investigations related to plans of care

Lead in the development and updating of referral guidelines and policies for the service

Use highly developed communication skills to effectively communicate with colleagues, patients and their relatives/carers, making reports and liaising as required with medical staff and other members of the multidisciplinary team both verbally and in writing

Use professional judgement to act as an advocate for patients to ensure a patient focused approach to the delivery of care. Support and enable patients and carers to make informed decisions relating to their treatment and management. Escalate any concerns via the doctors / professional structures as required

Work towards safe and timely discharge and/ or transfer of care of patients from or between hospitals and services and healthcare professionals, ensuring barriers to discharge / transfer are identified and acted upon appropriately

Report and raise concerns related to Safeguarding, accessing advice and support as required. Be conversant with Trust policies for safeguarding including the assessment of mental Capacity, and consideration of Deprivation of Liberty Safeguards (DOLS) and the application of the principles of Prevent (counter terrorism awareness)

Ensure effective and accurate verbal or written handover of patients between healthcare professionals

Ensure that high standards of all documentation are maintained, with accurate, complete and up-to-date information regarding patient care are kept in accordance with Trust standards

ACUTE & CHRONIC DISEASE MANAGEMENT:

Work with all GPs and other members of the practice as directed in developing protocols for diagnosing and managing minor illnesses, and for chronic disease management etc.

Work with the nursing, clinical pharmacy, and medical team in providing pro-active management of patients with chronic diseases in accordance with NSF and local requirements.

Proactively identify, diagnose, and manage treatment plans for patients with and at risk of developing a long-term condition (as appropriate).
