Job summary

This is a new role within our PCN for an experienced ACP to support patients within our care homes, delivering high quality planned and proactive care to avoid unplanned hospital admissions, as well as working within our GP practices alongside primary care staff and with our home visiting team. This role will allow the successful candidate to spend the quality time needed with patients to ensure a personalised care approach is taken when discussing and treating their healthcare needs.

To work within their professional boundaries providing first point of contact for patients with undifferentiated and undiagnosed health care needs, including telephone triage and face to face consultations. There may be the need to perform home visits as well along with acting as the link and providing care for patients residing in the PCNs Care Homes.

To work collaboratively with the healthcare team, meeting the needs of the patient whilst demonstrating critical thinking in the clinical decision-making process. This will include history taking, clinical assessment and diagnostic skills in partnership with the practices in developing the role further.

Will have a knowledge of long-term conditions.

Independently prescribe (where applicable to the role) and review appropriate medications in accordance with local policies and evidence based medical practice and work collaboratively with the wider team to support patients and to promote medication adherence.

Main duties of the job

To order investigations, follow-up results and arrange appropriate management including seeking advice where appropriate.

Providing face-to-face, acute medical treatment for care home residents seeking a care primary care service.

To work with a multidisciplinary team including community and voluntary sector to improve patient care.

Actively taking a personalised care approach and population centred care approach to enable shared decision making with the presenting person.

To support the GP practices with the ongoing management of care home residents by reviewing amending residents RESPECT paperwork and Personalised Care and Support Plans as well as undertaking care home rounds and supporting with Multi-Disciplinary Team meetings.

The successful candidate will have a Post Graduate Degree Qualification in their field, Clinical Practice or equivalent qualification, with evidence of academic study and practical experience of situations that are typically encountered in a primary care setting.

You will be registered with a professional body. It is a requirement for the candidate to have attained a post graduate qualification in Prescribing or be working towards qualification.

Wakefield North Primary Care Network (PCN) is looking to recruit an Advanced Practitioner workforce. We are a group of six GP practices based in the Eastern side of the Wakefield District (Riverside Medical Centre, Henry Moore Surgery, Tieve Tara Medical Centre, Station Lane Medical Centre, St Thomas Road Surgery and Newland Surgery).

Conexus Healthcare CIC is the Confederation for general practice in Wakefield, we have four areas of work.

1. Delivery of high-quality public facing Health and Wellbeing services across the Wakefield District: We develop, manage and deliver health and care services that support the population of the whole Wakefield district

2. Delivery of Training, Development and Consultancy support across the United Kingdom: We design and deliver a range of training courses, development activity and provide advice and guidance that supports general practices and other primary care organisations.

3. Supporting Our Practices and Primary Care Networks (PCNs): We support general practices and PCNs across the Wakefield district to be more resilient, sustainable and deliver better patient care and care for the health needs of their populations.

4. Providing a voice and influence for General Practice in the Health and Care System: We provide a forum to develop a unified voice for General Practice and support general practices to cooperate and work collaboratively with the wider health and care sector partners in the district.
