Job summary

The vacancy is with Pontefract and Knottingley PCN which is made up of four GP Practices, Ashgrove Medical Centre, Friarwood Medical Centre, Northgate Medical Centre and Stuart Road Medical Centre serving a population of approximately 48,000 registered patients. The practices are located across Pontefract and Knottingley district.

The post holder will play an important role within the PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

The role will involve working with GPs and practice teams and also the Care Home Team to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.

Main duties of the job

The successful candidate will work across the four Practices as part of Pontefract & Knottingley Primary Care Network (PCN). They will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing high quality support.

They will be an integral part of the PCNs multidisciplinary team, working alongside social prescribing link workers and health and wellbeing coaches to provide an all-encompassing approach to personalised care.

A part of the role will be working with the Care Home team which plays an important part in working with the elderly and learning disability residents in the local care homes, including those with long term conditions. You will support in providing routine care for these residents. The team acts as a key point of contact between care homes and Practices. Building relationships with professionals, clients, residents and family members is essential.

The team also supports the Clinical Director and member practices in coordinating key activities including access to service, advice, information and ensuring health and care planning is timely, efficient, and patient centred as well as supporting digital initiates.
