Job summary

West Wakefield PCN is looking for a PCN Service Development and Operations Lead to support our developing PCN Management Team. The postholder will work closely with the existing PCN Business Manager, Team Leaders, and Clinical Directors to help deliver the strategy and vision of the Primary Care Network and support the staff team.

West Wakefield PCN is a PCN made up of five practices on the West of the district (Chapelthorpe Medical Centre, Middlestown Medical Centre, Orchard Croft Medical Centre, Ossett Surgery, The Lupset Health Centre). Our aim is to offer patient centred health care which is high quality, cost-effective and fully integrated with our local partners to ensure a sustainable health service for our local community.

This is one role that requires working across all 5 practices.

Main duties of the job

This new role will support the existing Business Manager to drive the development of an innovative and sustainable PCN.

The Service Delivery and Operations Lead will support delivery of patient services provided by the PCN, including the delivery of a workflow service, COVID-19 vaccination programme, LTC housebound team, and LTC Diagnostic Hub. The post holder will provide non-clinical oversight and governance of the programmes and ensure the efficient delivery of all services.

Areas of responsibility will include operational control, budget and performance, monitoring and reporting, rota management, leadership, change and service improvement, audit and evaluation, ensure quality, risk and governance is adhered to, and relationship development with our member practices and other providers. You will also be required to support delivery of the Access Plan by helping to develop IT systems and processes, and supporting effective patient communications including developing and maintaining a website, intranet, patient communications through social media, and waiting room information.

This role will support all PCN Member practices of the West Wakefield Network. You will be instrumental in enabling the PCN to develop and expand services to incorporate new opportunities.

About us

This post will be employed by Conexus Healthcare CIC, however the successful candidate will be deployed within the PCN. Conexus Healthcare CIC is the Confederation for general practice in Wakefield, we have four areas of work.

1. Delivery of high-quality public facing Health and Wellbeing services across the Wakefield District: We develop, manage and deliver health and care services that support the population of the whole Wakefield district.

2. Delivery of Training, Development and Consultancy support across the United Kingdom:

We design and deliver a range of training courses, development activity and provide advice and guidance that supports general practices and other primary care organisations.

3. Supporting Our Practices and Primary Care Networks (PCNs):

We support general practices and PCNs across the Wakefield district to be more resilient, sustainable and deliver better patient care and care for the health needs of their populations.

4. Providing a voice and influence for General Practice in the Health and Care System:

We provide a forum to develop a unified voice for General Practice and support general practices to cooperate and work collaboratively with the wider health and care sector partners in the district.

Built within primary care, for primary care, Conexus Healthcare CIC works with local GPs and their practices to champion investment in and deliver effective primary care at scale.

Main Duties and Responsibilities

Understanding of clinical, operational and financial requirements.

Service delivery needs to be patient and customer focussed with service outcomes in line with agreed plans and management of some patient complaints (not practice-level complaints) and suggestions.

To be on top of all service delivery processes and outcome measures in all services across the business at all times such that potential issues are spotted quickly and early intervention is made as necessary or escalated to SMT as required.

To ensure, in collaboration with the PCN Member Practices, the PCN Clinical Director and the PCN Coordinator, that service delivery goals are effectively planned and delivered and that the level of service delivery understanding across the PCN is well-founded and appropriate.

To implement and manage an effective service delivery planning, management and risk control regime; to provide evidenced assurances to colleagues that service delivery is safe, patient-focused and delivering outcomes which are in line with agreed short and long-term plans.

Collaborate with colleagues in any audit processes, review and disseminate results, and take appropriate action in response to audit findings.

To build and manage relationships with colleagues, commissioners and service partners to support service delivery and new business ambitions.

To revisit service delivery operating models and refine them as needed on an ongoing basis through structured test and learn approaches. To design and build operating models for new business as required from time to time.

To ensure that these new operating models deliver service outcomes which are in line with agreed plans.

Measures of success including quality, outcomes, activity, productivity, and compliance with financial plan.

Operational Performance

Responsible for the day to day efficient and effective operational management of service areas, linking appropriately with clinical and multi-disciplinary staff in all matters affecting the delivery of these services.

Resolve day-to-day performance issues within the Services, responsible for early identification of issues and creating the plan for resolution. Implement longer term solutions.

Ensure effective processes and procedures are in place to monitor and track performance within the Services. This includes contingency planning. Ensure compliance with all relevant policies.

Reporting regularly to the PCN and commissioners on service performance and operational standards.

Interpret data, so the PCN have access to timely and accurate information on all key performance indicators.

Responsible for on call manager duties as agreed, taking necessary action to maintain operational effectiveness and supporting all staff in resolving all issues.

Leadership

Deliver an efficient and effective service by actively supporting and supervising teams.

Responsible for developing and motivating staff through effective personal leadership, ensuring views and decisions are communicated both up and down the PCN management structure

  1. Support and mentor staff, both as individuals and as team members with regular 1:1s and Team Meetings
  2. Provide operational support and leadership to team leaders within the PCN to support implementation of services
  3. Utilise routine data reports to facilitate adequate monitoring of PCN and Team performance. Using relevant data to inform 1:1 staff meetings.

Operational Controls

Ensure effective processes are followed for resource deployment within the Services

Responsible for the development of clinical and non-clinical rotas to maximise use of available clinical time; plan clinical availability over long term; undertake regular clinical workload reviews.

Anticipate staff shortages in admin, clinical and operational areas and coordinate action from relevant leads to minimise the impact of these on service delivery.

Undertake designated tasks to achieve workforce plans which anticipate service changes and developments and which address recruitment, retention and workforce supply issues.

Manage stock and levels of supply

Strategy, Change and Service Improvement

Contribute to the strategic development of the Service.

Identify opportunities for service redesign, focusing on a patient centred approach to delivery; contribute to plans which improve and develop services.

Finance

Manage any delegated budget responsibilities for area, adherence to agreed budget plans, identifying efficiencies and implementing plans to reduce costs or make appropriate investments.

Assist and support with the identification of cost pressures, service development and cost improvement plans and support with the development of business cases to meet requirements.

Quality, Risk and Governance

Ensure that the outcomes of root cause analysis, and serious incident investigation issues are implemented.

Undertake complaint investigation as appropriate in conjunction with the wider PCN Team.

Premises and equipment

Ensure best use of shared premises for services is implemented by the PCN.

Maintain systems for ensuring adequate stocks of stationery, clinical and other supplies. Review on a regular basis to ensure cost effectiveness.

Ensure equipment is compliant with maintenance schedules.

Information and Management Technology

Establish systems for the flow of patient records around the service and the integration of information into patient records; ensure local and/or national guidelines are adhered to.

Oversee all systems for data security and protection, including back-up; ensure compliance with current legislation and Caldicott requirements.

Analyse and reconcile output of data in line with service reporting measures.

Assist with and/or lead the process of project management for new or extension of existing services; plan and execute implementation.

Monitor all requests made under the Data Protection Act 1998 and Access to Health Records 1990, and Freedom of Information ensuring that all requests are met.

Abide by the organisations code of conduct and Caldicott principles in confidentiality at all times.

Development of clinical systems and processes including templates, batch reporting, reporting, and protocols

  1. Be an owner for PCN Teams in Office 365, a system manager for the PCNs SystmOne units,
  2. Be a smart Card sponsor and manager to approve IT access
  3. Manage user access to all PCN systems and hardware
  4. Create clinical systems and processes.

Recruitment

Lead on developing appropriate job specifications and advertising positions.

Work closely with Conexus HR Support to ensure that all PCN recruitment, onboarding, and staff management processes are in line with Conexus policies.

Develop and lead in onboarding new staff and ensuring that they have a suitable PCN Induction programme.

Retention & Performance

Work in collaboration with the PCN Management team to develop the PCN workforce leading on recruitment, deployment, mentoring and appraising.

Support practices with planning for PCN Network Target Sessions

Identify training and development needs through staff and team leader feedback, supporting staff with proposals for education and training.

Support with development of quality monitoring and support mechanisms for any PCN staff and relevant subcontractors.

Communicate effectively within the team to ensure others can carry out their work efficiently and in compliance with professional standards and legislation.
