Job summary
We are Brigantes Primary Care Network and we are looking for you to join our expanding multidisciplinary team to help develop integrated care and fulfil the requirements of the PCN objectives. We currently have a team of two Physicians Associates, we are hoping to make you our third!
You will be responsible for the delivering of patient-centred care. You will be required to work autonomously in the practices, providing a range of services such as assessment, diagnosis, treatment, telephone triage, clinical decision-making and referring patients appropriately, as well as supporting the management team in the reviewing of clinical policy and procedure when required.
Your desire to effect positive change and compassionate nature will make you successful in this role, alongside your organisational skills and attention to detail.
Main duties of the job
You will be required to work autonomously and will have the key responsibilities in delivering health services within the PCN. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
Providing care for the presenting patient from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care.
Demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients.
Work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedures.
You will also review and act appropriately on blood results and referrals/correspondence regarding their patients.
Carry out audit work and long-term disease management / monitoring as per PCN DES requirements.

About us
Primary Care Networks (PCN) are groups of GP practices working closely together - along with other healthcare staff and organisations - providing integrated services to the local population.
Brigantes is situated in the west of Wakefield providing healthcare services to around 50,000 patients. The member practices use a collaborative approach including the use of additional roles to strengthen communities and reduce inequalities in health. The additional roles are key to the PCNs achieving the IIF targets from the Network Contract Directed Enhanced Service.
We currently host a number of Additional Roles across three PCNs, such as Pharmacists, Pharmacy Technicians, and Care Coordinators. There are plans for Nursing Associates and Trainee Nursing associates and paramedics to join within the next year. Social prescribing and First Contact Practitioners are provided by outside organisations.
