Job summary

Trinity Health Group PCN is looking to recruit Health and Wellbeing Coaches to join our PCN team. You will be joining an established Wellbeing Team made up of an existing Health and Wellbeing Coach, several Care Co-ordinators, a community Psychiatric Nurse all currently employed by the PCN and a Social Prescriber (currently Live Well Wakefield). This is an exciting time to join our dynamic Primary Care team and shape the future of patient care at the first points of contact.

Main duties of the job

Health and Wellbeing Coaches are an integral part of the general practice team working in each practice across the PCN. The post holders will work in partnership with their clinical and non-clinical colleagues, management support and the wider PCN to ensure the role delivers the best possible outcomes for our patients. Health and Wellbeing Coaches support people develop their knowledge, skills and confidence in managing their health and care, to improve their health outcomes and quality of life. Health coaches do this by using coaching techniques and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives.

Health and Wellbeing Coaches may work with patients over the phone, by video conference, or face-to-face. The role however is varied, and may include;

· Supporting self-management education

· Peer support

· Caseload management

· Facilitating group consultations.

· Identify opportunities and developing and delivering project recommendations for services and activities needed for active lifestyles that contribute to health outcomes, community safety and independent living in the Primary Care Network.

Key Responsibilities

  • Take referrals from the wider practice team including GPs, Practice Nurses, Social Prescribers, Clinical Pharmacists and Physiotherapists who will have identified patients that meet the criteria for Health and Wellbeing coaches.
  • Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement and achieve their personalised health and wellbeing objectives. This will include sitting in consultation with a patient and providing them with lifestyle medicine advice, using motivational interviewing techniques, setting goals that are personalised to their individual needs. It could include dietetics and nutrition, exercise, sleep and stress amongst other areas of lifestyle medicine advice but always focussed on personal coaching and motivation.
  • Ensure all interventions and coaching are designed to empower patients to be active participants in their own healthcare, empower them to manage their own health and wellbeing, and live independently. Focussing on ‘what matters to them’.
  • Support and deliver group consultations, with clinical and non-clinical colleagues, implementing new approaches to health across primary care and supporting individuals to access peer support.
  • Work with the broader MDT to maximise the support available to patients, including the social prescribing team to connect patients to community-based activities which support them to take increased control of their health and wellbeing.
  • It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the coach role – e.g. when there is a mental health need requiring a qualified practitioner.
  • Work with social prescribing colleagues to draw on and increase the strengths and capacities of local communities, enabling local VCSE organisations and community groups to receive social prescribing referrals. Ensure they are supported, have basic safeguarding processes for vulnerable individuals and can provide opportunities for the person to develop friendships, a sense of belonging, and build knowledge, skills and confidence.
  • Work together with all local partners to collectively ensure that local VCSE organisations and community groups are sustainable and that community assets are nurtured, by making them aware of small grants or micro-commissioning if available, including providing support to set up new community groups and services, where gaps are identified in local provision.
  • Work with the Wellbeing Team to promote Health and Wellbeing of Practise/NHS staff, which may involve planning and delivering group facilitation to colleagues.

  • Give people time to tell their stories and focus on ‘what matters to me’.
  • Build trust and respect with the person, providing non-judgemental and non-discriminatory support, respecting diversity and lifestyle/cultural choices.
  • Work from a strength-based approach focusing on a person’s assets.
  • Be an approachable, evidence-based source of information about wellbeing and prevention approaches.
  • Work with individuals to co-produce a simple personalised support plan – based on the person’s priorities, interests, values and motivations – including what they can expect from the groups, activities and services they are being connected to and what the person can do for themselves to improve their health and wellbeing.
  • Where appropriate, physically introduce people to community groups, activities and statutory services, ensuring they are comfortable. Follow up to ensure they are happy, able to engage, included and receiving good support.

Referrals

  • Promote health coaching, its role in supported self-management as a part of personalised care, in addressing health inequalities and the wider determinants of health.
  • As part of the PCN multidisciplinary team, build relationships with staff in GP practices within the local PCN, attending relevant multidisciplinary meetings, giving information and feedback on health coaching.
  • Seek regular feedback about the quality of service and impact of health coaching on referral agencies/origins.

Gathering and Reporting Information

  • Complete relevant wellbeing questionnaires and survey’s with patients to provide data and showcase the effectiveness of the Health and Wellbeing Coach role in the NHS and the impact coaching has on patient’s lives.
  • Work closely within the multidisciplinary team and with GP practices within the PCN to ensure that the relevant SNOMED codes to record activity are inputted into clinical systems (as outlined in the Network Contract DES), adhering to data protection legislation and data sharing agreements.

Supervision/Personal/Professional development

  • You will be assigned a Line manager and an onsite supervisor at the practices where you are based. Annual Appraisals and Monthly 1-2-1’s will be scheduled with your line Manager/ Supervisor.
  • The post-holder will participate in any training programme implemented by the Practice as part of this employment.
  • Participation in an annual appraisal, including taking responsibility for maintaining a record of own personal and/or professional development.
  • Work with your line manager to access regular ‘clinical supervision’, to enable you to deal effectively with the difficult issues that people present.

Personalised Care Institute (PCI)

  • The Personalised Care Institute (PCI) was launched in September 2020. It is a virtual organisation accountable for setting the standards for evidence-based training in personalised care in England.
  • Training for health and wellbeing coaches is accessed through PCI accredited or assured training providers. These are listed here.
  • On completion of training learners will be registered with the PCI institute and receive a completion certificate. In addition, the core curriculum for personalised care is available here and may be a useful reference point for health and wellbeing coaches.

Miscellaneous

  • Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner.
  • Identify opportunities and gaps in the service - and review risks and issues that could impact on service delivery - and provide feedback to continually improve the service and contribute to business planning.
