Job summary

The Long Term Condition Home Visiting General Practice Assistant (GPA) will join a team within Five Towns PCN to support the care of house bound patients. With the support of the team the role will provide initial assessments and planning for patients with a holistic approach. Ultimately the aim is to improve the patients access/experience in practice.

General Practice assistants will operate under the supervision of highly experienced and qualified Nurses and at times other appropriately qualified clinicians. The assistant will undertake a training programme, if not already accredited, to obtain formal clinical sign off.

Main duties of the job

The following are the core responsibilities of the GPA. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels. You will work within your capabilities and:

Arrange referrals on behalf of the clinician both internally (to other clinicians or care coordinators/social prescribers/health and wellbeing coaches) or externally (to the hospital or community clinics)

Responsibility for the processing of urine specimens

Supporting clinicians with administrative tasks

Vaccines, injectables, drug stock management

Complete simple clinical observations/investigations as directed, such as dipstick urine, taking blood pressure, ECGs, phlebotomy etc.

Complete basic (non-opinion) forms and core elements of some forms for the GP to approve and sign (such as insurance forms, mortgage, benefits agency forms etc.)

Help liaise with external agencies (such as arranging admissions)

Supporting with QOF reviews

Supporting the recall system for Long Term Condition reviews
