Trinity
Health Group PCN is looking for an Outreach Nurse with experience of long-term
condition reviews and management to join our Outreach Team. The postholder will
work with our existing LTC Outreach Nurse forming an innovative team developing
new ways of working. The postholder with focus on completing LTC reviews for
housebound patients plus being a key member of the teams delivering on our
Health Inequalities Project. It is likely that future project work will involve
the postholder and there will be opportunity to develop unique services and
pilots.

Main duties of the job
With
the NHS Long Term plan in mind the team will be working on several projects
such as;
- Reducing Health Inequalities by targeting
specific patient cohorts with interventions. This will include setting up pop
up clinics in areas of social deprivation, traveller sites and working with
community/religious groups making our services more accessible for
patients.
- Targeted
support for patients with Cardiovascular Disease and
Diabetes. Completing Long -Term Condition Reviews, monitoring, and taking
regular observations. Recognising ways in which these patients require support
and interventions from multiple practitioners and making every contact
count.
- The
Outreach Team work closely with our Wellbeing Team to provide a coordinated
approach to LTCs within Care Homes, Dementia and those with multiple
LTCs.
- The successful candidate will have a can-do attitude and be
flexible to suit the needs of the business and the projects that we are
delivering.
The
Long Term Condition Outreach Nurse will provide and maintain a high standard of
nursing care for patients with a particular focus on those who are housebound.
Focusing upon supporting patients to be healthy, monitoring of long-term
conditions, health prevention and screening activities. They will work
collaboratively with the General Practice team to meet the needs of patients,
supporting the delivery of policy and procedures, and providing nurse
leadership as required.

Qualifications
Essential
- Qualified Registered General Nurse or equivalent with current NMC Registration
- Good standard of general education and good standard of spoken and written English language
- Evidence of continuing professional development within the last 12 months
- Diplomas in chronic disease management e.g. Diabetes, Asthma, COPD etc
Desirable
- Masters Degree in relevant subject area
Experience
Essential
- Substantial post-registration experience Practical experience of clinical procedures as per job profile / advertised requirements
- Experience of planning and managing clinics
- Experience of working with housebound patients and hard to reach groups
- Experience of wound management
- Experience of working in General Practice
- Experience of providing respiratory clinic or equivalent
- Experience of working in a MDT setting
- Experience of documenting and record keeping of clinical information
- Experience in primary care
