Job summary

Trinity Health Group PCN is looking for a PCN Access and Operations Lead to support our developing PCN Management Team. The postholder will work closely with the existing PCN Business Manager, Management Team and Clinical Directors.

Please note this is not an Agenda for Change Contract.

Interviews will be held on or around 19th July 2022.

Main duties of the job

Trinity Health Group PCN has employed a large number of staff over the last 2 years, a significant proportion of these staff are clinical. We recognise that we now need to build in a supportive management structure in order to continue our excellent work and continue to grow and develop. This new role will support the existing Business Manager to drive the development of a sustainable PCN.

Trinity Health Group is a PCN made up of four practices on the west of the district (Trinity Medical Centre, Maybush Medical Centre, Crofton and Sharleston Medical Practice and Warrengate Medical Centre). This post will be employed by Conexus Healthcare, however the successful candidate will be deployed within the PCN. Conexus Healthcare is the Confederation for general practice in Wakefield, we have four areas of work

1. Delivery of high-quality public facing Health and Wellbeing services across the Wakefield District.

2. Delivery of Training, Development and Consultancy support across the United Kingdom

3. Supporting Our Practices and Primary Care Networks (PCNs)

4. Providing a voice and influence for General Practice in the Health and Care System

The post will evolve as we develop our services but the postholder will be required to undertake the following duties:

PCN contracts and DES Specification

· Work alongside the Business Manager, the PCN Management Team, Clinical Directors and PCN Practices and Board to; develop ideas and provide suggestions as to how they can deliver the DES specification, drive the development of the network and support and facilitate the implementation of services in line with Network Contract Directed Enhanced Service specifications (DES), and any other initiatives the PCN is pursuing.

· Support the growth of the PCN and the delivery of services in line with the Network DES and the Investment and Impact Fund (IIF). Ensuring the required outcomes are achieved and ensuring all team members understand and capture the required data set.

· Represent the PCN in cross organisation meetings when agreed e.g. ICS meetings

· Develop systems to ensure that documentation is signed and in place e.g. data sharing agreements, secondment agreements etc.

HR and People Management

Organisation

· Have overall responsibility for the management of staff rotas to ensure service levels are maintained as far as possible.

· Support teams to ensure information is recorded accurately to support Practices with Clinical Audits

· Ensure that the PCN has adequate policies and operating procedures to ensure consistent approaches between teams and team members. Ensuring these are communicated, implemented, and regularly audited for compliance.

· Review policies and procedures to ensure they are up to date and effective, implementing effective version control systems

· Support the Business Manager with PCN finances ensuring services are delivered within budget and all expected PCN income is received.

Recruitment

· Lead on developing appropriate job specifications and advertising positions.

· Work closely with Conexus HR Support to ensure that all PCN recruitment, onboarding, and staff management processes are in line with Conexus’ policies.

· Develop and lead in onboarding new staff and ensuring that they have a suitable PCN Induction programme.

Retention & Performance

· Work in collaboration with the PCN Management team to develop the PCN workforce leading on recruitment, deployment, mentoring and appraising.

· Support practices with planning for PCN Network Target Sessions

· Ensure that all PCN staff have adequate provision for training and development that supports their role and benefits patients.

· Identify training and development needs through staff and team leader feedback, supporting staff with proposals for education and training.

· Support with development of quality monitoring and support mechanisms for any PCN staff and relevant subcontractors.

· Understand and identify risk and implement necessary mitigation actions and report significant risks and proposed actions through DATIX and other local reporting systems.

· Identify and deal quickly, fairly, and consistently with poor performance in line with HR policy.

· Motivate and empower others to achieve set goals, create an open and honest environment and demonstrate high standards of morals and integrity.

· Communicate effectively within the team to ensure others can carry out their work efficiently and in compliance with professional standards and legislation.

Line Management

· Be responsible for leading, motivating and effectively managing the relevant Team Leaders, relevant subcontractors, and admin support within the PCN by providing excellent managerial leadership.

· Support and mentor staff, both as individuals and as team members with regular 1:1s and Team Meetings.

· Provide operational support and leadership to team leaders within the PCN to support implementation of services.

· Utilise routine data reports to facilitate adequate monitoring of PCN and Team performance. Using relevant data to inform 1:1 staff meetings.

IT Systems and Hardware

· Be the lead contact for PCN IT management.

· To ensure that documentation (electronic and paper-based) is appropriately stored, shared and destroyed. Ensuring that only relevant staff have access to specific documents and data.

· Lead on the migration of the PCN shared drive to SharePoint, liaising with colleagues at THIS and the ICS/Wakefield Place.

· Have overall responsibility for the set up and implementation of the PCN Intranet through SharePoint, encouraging collaboration and utilisation IT systems to support the growing PCN.

· Support PCN staff to access relevant systems and IT support as and when required.

· Be an owner for PCN Teams in Office 365, a system manager for the PCN’s SystmOne units,

· Be a smart Card sponsor and manager to approve IT access

· Have overall responsibility for setting up and monitoring PCN IT – including (but not exclusively) phone systems, PCN Mobile phone accounts, Staysafe app, TeamNet and any future PCN IT systems and hardware.

· Manage user access to all PCN systems and hardware.

Premises and Estates

· Be the main point of contact for premises issues and external and internal suppliers/providers such as NHS Estates.

· Ensure that all premises leased by the PCN are fit for purpose and adequately resourced in order to provide the best possible working environment and deliver excellent service to our patients and practices, whilst being mindful of public spending.

· Ensure that working areas are fit for purpose by ensuring the following:

o Suitable furnishings and decoration

o Safe storage for PCN and Staff property

o Appropriate insurance, risk assessments etc

o Appropriate methods for team communication and collaboration

o Necessary IT hardware installations

Purchasing

· Work alongside the Business manager, to purchase items required for the PCN and the PCN staff to carry out their work safely and effectively.

· Working within the set parameters for spending to ensure that resources are supplied in a timely manner within budget constraints.

· Maintain an asset register and ensure that equipment is utilised effectively.
