Job summary

An exciting opportunity has arisen for the employment of a Roving Practice Nurse to join Conexus Healthcare. As a Practice Nurse you will be working within Trinity Health Group Primary Care Network (PCN).

PCNs are groups of GP practices working closely together to provide integrated services to the local population. Trinity PCN is situated across West and Central Wakefield, providing healthcare services to around 55,000 patients. The member Practices use a collaborative approach including the use of roles like this one to strengthen communities and reduce inequalities in health.

The Roving Practice Nurse will provide and maintain a high standard of nursing care for patients with a particular focus on those who are housebound or hard to reach. Focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the General Practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Main duties of the job

Visit patients who are hard to reach or who would benefit from out-reach to provide a Long-Term Condition service.

Diagnose as appropriate and manage both acute and chronic conditions, integrating both drug-and nondrug-based treatment methods into a management plan

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.

Work with patients in order to support compliance with, and adherence to, prescribed treatments.

Provide information and advice on prescribed or over-the-counter medication, medication regimes, side-effects and interactions

About us

Conexus is the GP Confederation in the Wakefield District. In addition to training staff in practices, and providing clinical services, Conexus provides support to local PCNs by hosting staff and supplying support services to them. The successful candidate will be an employee of Conexus Healthcare and their work will be PCN-based under the direction of the PCN Business manager and Clinical Directors.

AS part of Trinity Health Group PCN you will be joining a larger team of PCN staff. These currently include the Pharmacists and Pharmacy Technicians line managed by the Pharmacy Team Leader. Care Coordinators, Health and Wellbeing Coaches, and a Mental Health Nurse line Managed by the Wellbeing Team Leaders. We have plans to recruit Paramedics, Trainee Nursing associates and Healthcare Assistants. Overseeing all of these is are the PCN Business Manger and their Assistant. All of the PCN staff have been employed within the last 18months, so it's an exciting time to join our team and trail blaze new ways of working in primary care.

Implement and evaluate individual treatment plans for patients with a known long-term condition.

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies and provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions.

Assess and care for patients presenting with uncomplicated wounds.

Initiates and carries out programmes of health screening. Contributes to profiling of the practice population. Supports Practices to achieve Quality and Outcomes Framework and the Impact and Investment Fund Targets.

Enables, encourages and supports individuals, families and groups to address issues that affect their health and social well-being.

Represents individuals and families interests when they are not able to do so themselves.

Provides healthy lifestyle and smoking cessation advice to individuals and groups.

Develops and maintains the health promotion material to support the roving team and link in with Practice materials.

Makes referrals to statutory and voluntary agencies as appropriate.

Support and advise women requesting information relating to family planning needs and manage health needs of women presenting for cervical cytology consultations.

Able to recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health.

Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary.

Have excellent communication and able to demonstrate the ability to communicate sensitively to ensure patients are fully informed and consent to treatment.

To be compassionate in meeting the needs of patients.

To work collaboratively as part of a Primary Care/PCN Team.

To be professionally accountable and responsible for patient care undertaking a range of clinical interventions either remotely or during face to face consultations.
