Main duties of the job
Job Description
To provide and maintain a high standard of nursing care to the practice population with focus in the delivery of evidence-based practice for patients presenting with long-term conditions and acute problems. This includes screening, assessment, treatment and education and taking an active role in supporting the practice to maximise achievement against local and national targets in respect of the GMS contract and Enhanced Services.
Key Responsibilities
- Conduct chronic disease reviews (e.g., diabetes, asthma, COPD)
- Administer vaccinations and immunisations
- Perform cervical screening (smear tests)
- Provide wound care and dressings
- Support family planning and sexual health services
- Deliver health promotion and lifestyle advice
Experience
Registered Nurse with up to date NMC Registration and must have worked in a Primary Care setting with knowledge of current policies, protocols and national targets.
