Job summary
Our much valued Lead Nurse is stepping down as part of her retirement plans and we are looking for an experienced practice nurse to step up and make their own mark in our practice. Ideally the successful candidate will already be an experienced practice nurse with management/leadership experience (although this can be a supported development role). They must have a Chronic Disease qualification and experience in at least one speciality. They will be a core member of the leadership team whilst effectively managing a dedicated Nursing team. Working autonomously and also collaboratively, the Lead Nurse will contribute to the delivery of high-quality patient care through clinical excellence and quality improvement initiatives.
The Family Practice is located close to the centre of Bristol, with beautiful gardens, a large car park and an incredible team. We are a Good rated practice which prides itself on being a training practice and working collaboratively in our highly functioning PCN.
The successful candidate will be working a total of 32-37.5 hours a week however you will be expected to work occasional Extended Access hours in addition to core hours this will be on a rotational and infrequent basis.

Main duties of the job
The Lead Practice Nurse will be responsible for the management of the practice nursing team, consisting of nurses and HCAs.
The Lead Practice Nurse will be responsible for several clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
The Lead Practice Nurse will be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

The partnership consists of 5 partners whose focus is on good patient care as well as the welfare of the team.
We are part of a highly functioning PCN and collaborate to offer the best care for our patients through joint ventures. It also organises socials and regular lunches for the teams which prove very popular.
The patients especially appreciate our treatment room team (judging by the amount of cards, cakes and chocolates available!) and many team members who love to bake. We have several runners in the practice that you could join to burn off those calories if you are so inclined and if you are not (that's fine too), others go for a brief walk or sit out in our garden.
