We are looking for an experienced, qualified Practice Nurse to join our team.

As a NMC Registered General Nurse, the right applicant will need to be trained in a wide range of nursing duties including the management of patients with long-term conditions in particular those patients with Diabetes, providing and delivering primary care interventions and care effectively and efficiently

Effective communication with patients, carers and all members of the MDT

Have the ability to priorities and complete daily work in line with clinIcal / patient need and practice priorities

Job Summary:

Working as part of the practices multidisciplinary team who provide care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

Main duties of the job

Job Responsibilities:

Duties include [but not limited to]:-

Provide routine nursing care to patients as required in accordance with guidelines and protocols undertake and record:

  • Maintenance of accurate and legible patient records in line with professional and organisational requirements
  • Participates in clinical audit and the implementation of changes as identified
  • Ensure that any complaints or significant events are identified and reported in accordance with organisational policy and as expected professional practice
  • Ensure clinical practice is safe and effective and remains within competence levels
  • Urinalysis and preparation of specimens for laboratory analysis
  • Provide child Immunisation
  • Cytology & HVS
  • Wound care
  • Doppler ultrasound measurements
  • Removal of Sutures
  • Routine adult and childhood vaccinations
  • Administer prescribed therapies such as contraceptive injections, vitamin B12 and depo provera
  • ECGs
  • Flu immunisations
  • Management of patients with Long-term conditions [preferably qualified in treating patients with Diabetes]
  • Assisting GPs with minor surgery (if HCA unavailable)
  • Requesting pathology tests, for example urine culture, swabs
  • Following agreed clinical protocols with referral to GPs as appropriate
  • Assist in and perform routine tasks related to patient care
  • Chaperone patients where necessary

Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

Participate in local initiatives to enhance service delivery and patient care

Support and participate in shared learning within the practice

Continually review clinical practices, responding to national policies and initiatives where appropriate

Participate in the review of significant and near-miss events

Support the clinical team with all safeguarding matters, in accordance with local and national policies

Deliver opportunistic health promotion where appropriate
