Job
Summary:
The post holder is responsible for the
delivery of practice nursing services care to the practice population. They will deliver care within the boundaries
of their role, focusing upon supporting patients to be health, monitoring of long-term
conditions, health prevention and screening activities. They will work collaboratively with the
general practice team to meet the needs of patients, supporting the delivery of
policy and procedures, and providing nurse leadership as required.
Duties and
Responsibilities:
Screening:
1) Undertake
opportunistic screening BP checks, height, weight, BMI, urinalysis, smoking
status.
2) Carry
out new patient checks.
Health
Promotion:
1) Recognise
patients readiness to change in supporting their attempts to change unhealthy
behaviour.
2) Provide
healthy eating advice to individuals or groups, including the underweight.
3) Provide
smoking cessation advice individually in opportunistic consultations.
4) Provide
individual advice to patients about increasing activity or exercise.
5) Offer
advice on recommended safe limits of alcohol consumption.
6) Develop
and maintain the Health promotion material within the practice.
7) Refer
patients as appropriate.
Vaccination
and Immunisation:
1) Administer
injections to children and adults under an individualised prescription.
2) Patient
Group Direction or Patient Specific Direction.
3) Ensure
safe storage, rotation and disposal of vaccines and drugs.
Infection
Control:
1) Support
patients and colleagues in adopting sound infection control measures.
2) Apply
infection control measures within the practice accordingly to local and
national guidelines.
This
includes:
Universal
hygiene precautions.
The
collection and handling of laboratory specimens.
Segregation
and disposal of waste materials.
Decontamination
of instruments and clinical equipment.
Reporting
and treatment of sharps injuries.
Dealing
with blood and body fluid spillages.
Educate
colleagues in adopting sound infection control measures.
Emergency
Management:
1) Is
competent in anaphylaxis and resuscitation techniques.
Child
Health / Child Protection:
1) Recognise
the signs, symptoms and categories of child abuse.
2) Understand
the importance of recognition, observation, documentation and communication.
3) Is
aware of child health procedures, statutory local procedures and points of
referral.
4) Has
knowledge of other practitioners and agencies roles.
5) Understands
basic legal issues around child abuse.
Mental Health:
1) Recognises
and is aware of appropriate referral mechanisms for family violence, vulnerable
adults, substance abuse and addictive behaviours.
2) Recognise
psychological needs of patients presenting with depression or suicidal
tendency.
Mens
Health:
1) Raise
awareness and give basic advice on issues covering:
Testicular
self-examination
Incontinence
and prostatic disease
.Family
planning
. Sexual
health
2) Make appropriate
referrals
Family
Planning:
1) Advise
patients on available methods of contraception and refer patients as
appropriate.
2) Support
patients attending for emergency contraception and refers patients as
appropriate.
3) Preform interval health checks for patients
using hormonal contraception.
4) Teach
patients correct condom use.
5) Advise
patients on reducing risk of sexually transmitted diseases and refer patients
appropriately.
Womens
Health:
1) Advise
patients on the national and local Cervical Cytology and Mammography Screening
programmes.
2) Promote
breast self-awareness, demonstrate breast self-examination and provide
supporting literature.
3) Advise
the patients about reducing the risk of sexually transmitted disease and make
appropriate referral.
4) Take
swabs for microscopy as directed.
5) Give
patients basic advice about peri-menopausal and postmenopausal risk of
osteoporosis and heart disease.
6) Offer
health promotion / lifestyle advice to patients.
7) Advise
on basic incontinence management and make appropriate referrals.
Diabetes
1) Undertake
opportunistic screening for the diagnosis of diabetes.
2) Support
and provide education for diabetic patients.
3) Provide
support and emergency treatment for hypo/hyperglycaemic emergencies.
4) Make
appropriate referrals.
5) Contribute
to the maintenance of the practice diabetic register.
Respiratory
Disease Management:
1) Have
a basic understanding of asthma care; its diagnosis, asthma treatments and the
principals of self-management
2) Know
about the factors which contribute to the development of COPD.
3) Contribute
to the management of patients within the framework of national and local
guidelines for asthma and COPD.
4) Manage
emergency treatment of asthma / COPD exacerbations.
5) Teach
and advice patients how to use various inhalers and peak flow meters.
6) Contribute
to the management of the practice respiratory register.
7) Make
appropriate referrals.
Cardiovascular
Disease:
1) Contribute
to the implantation of National Service Framework.
2) Contribute
to the maintenance of the practice CHD register.
3) Advise
patients on the lifestyle management and drug compliance for reducing the risks factors of:
Hypertension
Angina
Myocardial
Infarction
Cerebrovascular
accident
Clinical
Practice Leadership:
1) Assist
in the clinical audit and the setting and monitoring of standards of care.
2) Teach
and direct others in clinical setting, including unqualified staff.
3) Act
as an assessor / mentor and teach to other staff.
4) Work
effectively in the team.
Clinical
Practice Development:
Participate
in evidence based practice and clinical supervision groups.

Additional:
1) Post
holder will be registered with the Nursing and Midwifery Council and provide
proof of yearly continuous registration to Practice Manager
2) Provide
current CRB certificate to Practice Manager
3) Provide
proof of immunisation status including Hep B.
