An exciting opportunity has arisen for a clinical pharmacist to join our friendly, supportive and progressive at Parkwood Surgery in Hemel Hempstead.
We are a large practice serving 17,500 patients in an urban setting with good transport links.
We are looking for an Independent prescriber and preferably with previous experience of working in General Practice although training would be given if needed.
The role will involve f2f medication reviews, management of long term conditions such as COPD, Asthma, medication queries and the medication reconciliation from hospital letters and discharge summaries. You will be well supported by our GP Prescribing lead and other members of the team.
Main duties of the job
We are looking for someone who is well organised, flexible, good at working as part of a team and also working alone. Up to date medication knowledge to be able to manage our patients long term conditions and monitoring associated with this.
You will be a key person in the overall medicines management and working with all of the GP's and especially the prescribing lead.
Knowledge of the Emis clinical system would be useful although full training will be provided.

Qualifications: Full registration with the GPhC

a Prescribing Pharmacist then suitable qualifications will be required.
Effect and maintain
membership of a recognised Medical Defence organisation approved by the
practice, commensurate with the post holders duties and at their own expense
throughout the period of employment. (The post holder is eligible to join the
practice group scheme with the MDDUS . If the post holder uses the Practice
Group scheme the surgery will cover the cost. If the post holder chooses to use
their own scheme the post holder will be responsible for the fees)

Job Summary:
The post holder will be required to provide medical
services to patients of the practice by way of surgeries, clinics and relevant
administrative work. Together with such other duties as are required of the
practice in providing such services under the provision of the Primary Care Act
1997 and which are reasonably delegated to the post holder by the practice.
Responsibilities
The services provided by this post will include:
- Establishing appropriate systems to manage common
chronic medical conditions such as asthma, hypertension and diabetes.
- Providing appropriate preventative health care and
advice within the context of primary health care.
- Attending primary health care team meetings and
other meetings as may be necessary for your duties.
- Auditing practice activity if requested.
- Medicines Optimisation, reviews of patient
medication regime, care home reviews.
- Managing repeat prescriptions.
- Adherence to local and national guidelines.
- Clinically related administrative and non-clinical duties
needed for the delivery of medicines management.
- Use of appropriate templates and codes as used for
QOF and Enhanced Services.
- The post holder is required to keep proper records
of all patient contacts. This includes computerised records on the practice
clinical system.
- The post holder would be required to complete all
paperwork and results relating to patients they have seen.
- The management of medication changes following
receipt of hospital discharge and outpatient clinics using the workflow system.
- Prescription of care including medication
- Attending surgery meetings when they coincide with
working pattern
- Providing a collaborative, supportive and holistic
approach to patients of the practice.

Maintain valid full GPhC registration.
The post holder will be expected to undertake necessary
CPD activity as required for revalidation.
