Job summary

We are pleased to announce an exciting opportunity for a Pharmacist to join our Primary Care services in Hemel. We are looking for a dedicated and motivated Pharmacist who seeks to make a significant impact inprimary care. This position offers a unique chance to become an integral part of our dynamic clinical team, where you will contribute to the delivery of high-quality patient care within our GP surgery.

Main duties of the job

As a Pharmacist, you will be at the forefront of patient care and support, working closely with GP practices to provide expert guidance on medicines, repeat prescriptions, and pharmaceutical care. You'll be an essential part of a multidisciplinary team, using your extensive experience to ensure the best outcomes for our patients. At the same time, we prefer a full-time commitment.

  • Providing expert services and support to patients registered with our Hub
  • Offering clear and accessible advice on medicines, repeat prescriptions, and general pharmaceutical queries
  • Assisting with prescribing, medicines optimization, and high-risk drug management audits across primary care sites
  • Using critical thinking and effective communication skills to assess and address patient needs
  • Prescribing urgent prescriptions and ensuring accurate re-prescribing using summary care record information
  • Identifying and addressing clinical risks in line with professional responsibilities
  • Keeping clear, accurate, and timely records of all consultations and advice given
  • Actively engaging with colleagues to deliver outstanding service
