Job summary

JOB SUMMARY

Work within the nursing team, providing administration support to identify cohorts of patients who require health reviews, blood tests, immunisations, cervical smear tests, etc. Contacting and following up patients to book them into appointments. Inputting information and results into patient records, running phlebotomy clinics, ordering clinical supplies and ensuring clinical rooms are stocked up.

Taking part in infection control procedures audits. Liaising and providing administration support to the Multidisciplinary Team (MDT), including assisting in referrals management. The post holder will need to be able to work well within a team, be able to prioritise a varied workload in a busy primary care practice and work in a methodical and accurate manner.

Main duties of the job

THE CARE CO-ORDINATOR WILL

Work within the nursing team to identify cohorts of patients who need to be contacted and booked into appointments for blood tests, annual long term condition reviews, immunisations, cervical smear testing, etc.

Provide clinical administration support to the wider MDT

Running phlebotomy clinics

Update patient records with information from outside agencies with recognised national coding system (SNOMED coding).

Record data from national screening programmes (eg: mammography, retinal screening, cervical screening, bowel screening) and follow up non-attenders.

Record data regarding new births, and book 6 week checks (mothers) and baby immunisation appointments

Co-ordinate administration to identify, invite and follow up patients for group consultations.

Use AccuRx Self Book to send text messages and booking links to relevant patients to improve access and ensure filled clinics.

Support and promote preventative healthcare by proactively contacting and having discussions with people eligible for public health programmes such as immunisation and cancer screening programmes.

Setting up and running clinical searches in order to monitor and focus care of target groups.

Using clinical systems to identify patients who need reviews, tests, etc.

Attending practice meetings.

Effectively manage own time, workload and resources.

Maintain accurate records at all times

Any other tasks that may be required by the Practice from time to time.
