Job summary
We are looking for a enthusiastic and confident individual to join our Care Navigator team as a call handler. This will mainly involve answering phone calls from patients, booking appointments in for them and signposting patient to other more suitable services as appropriate.
Applicants must be available to work between the hours of 07:15 10 19:00 Monday- Friday. The individual needs to be able to work both morning and afternoon shifts. This is a temporary position for 4 months to support our current team until the end of January 2022.
This is a part-time position and the hours can be discussed with the candidate but we would be looking for a minimum commitment of 16 hours a week.
Main duties of the job
- Maintaining and monitoring the practice appointments system by managing patient calls appropriately
- Signposting patients to other more suitable services as appropriate
- Processing personal and telephone requests for appointments, visits and telephone consultations and ensuring callers are directed to the appropriate healthcare professional
- Contact patients and external agencies as requested by members of the Primary Health Care Team.
- To provide cover for members of the reception team during periods of sickness and annual leave changing shifts where necessary
- Taking messages and passing on information
- Initiating contact with and responding to requests from patients, other team member and associated healthcare agencies and providers

The premises are owned by the partnership and is modern, purpose built and inviting to staff and patients alike.
We are a non-dispensing PMS practice, with a strong interest in both teaching and research. We are stable and successful business with 5 GP partners, 5 salaried doctors and an excellent team of advanced nurse practitioners, nurses and practice administration staff.
There is a strong team ethos within the practice and it is very important to the partners that this is maintained and developed. Continuous training and development are important and encouraged.
We are committed to a patient-centred methodology and have an open and friendly approach. We work well as a team and offer a full range of primary care services to patients.
