Job summary
An exciting opportunity has arisen for a
motivated individual to join our Coding Team, working 25 hours over four/five
days (dependant on applicant) Monday, Tuesday, Wednesday, Thursday and Friday,
plus holiday and sickness cover.
This role will be predominantly based in
Hessle but you must be available to work across all sites.

Main duties of the job
Processing of medical information and
correspondence through the Practice clinical system SystmOne, as per the
document processing protocol and coding protocol.
You will also be responsible for summarising
patient notes, using Snowmed codes to code the relevant information from
patients paper notes into the Practice Clinical System.
The ideal candidate will need to be able to
work well under pressure and cope with conflicting demands. You must possess
good communication skills and organisational skills. Experience of working in a
general practice along with coding experience is desirable but not essential.

Brough, Hessle, South Cave, Holme on Spalding Moor and Bubwith.

Job Purpose:
Processing of medical information and correspondence through the Practice' clinical system, SystmOne as per the document processing protocol. You will also be responsible for summarising patient notes, using Snowmed codes to code the relevant information from patient's paper notes into the Practice' clinical system.
Main duties and responsibilities:
Processing of medical information and correspondence through the Practice' clinical system, SystmOne into patient's records. Identifying actions, follow up, relevant information and clinical information for clinical coding. Understanding how certain clinical codes link into QOF and IIF targets.
To code all relevant information into the Practice' clinical system. Information should include all diagnosis, procedures, and appropriate values (results of tests). All codes should be recorded as per the clinic procedure date.
To ensure specific clinical codes are flagged in the major or minor summary and as a 'problem' where required. Please see Summarising Protocol.
Use Snowmed codes to code any appropriate data as requested through other workstreams received. Ensure coding is completed in accordance with data quality standards.
Responsible for pulling in the NHS mail daily and actioning any electronic requests.
Deletion of duplicate and incorrect codes as required. General maintenance of good data quality standards.
Highlight any errors in hospital letters and correspondence to the relevant GP or IT Lead.
Investigation of medical records and history to establish dates of diagnosis etc, where a date is not specified.
Support with the filing of Bowel, Cervical and Breast Screening results.
Work should be carried out in a quiet room at one of the Practices, and in accordance with confidentiality and data protection policies. No coding should be carried out at home, unless authorised by a manager.
Work safely at all times in accordance with legislative requirements and the Practice' policies and procedures.
Assist with other reasonable tasks, as and when required.
