We are looking for a proactive and confident communicator with excellent inter-personal and communication skills. A good understanding of health issues and the wider determinants of health and wellbeing is essential.

The main duties of the Community Support Worker will be:

The candidate right candidate will be able to work effectively as a supportive team plater as well as one one's own initiative.

a) To support people who are leaving hospital by providing personalised information and assistance in the use of technology to support them in self-managing their medications.

b) Work in partnership with the University of the West of England (UWE), BNSSG Integrated Care Board, the Local Authority Technology Teams and Sirona Care and Health, to deliver and evaluate a personalised medication support service.

c) Conduct home visits to advise on the most appropriate technology, support the installation and assess the effectiveness of the interventions.

d) Where necessary, work with NHS case managers to support service users discharge from hospital and facilitate follow-through in respect of their medication management.

e) Effectively manage and prioritise a caseload, visiting service users in hospital or at home, as required.

f) Deliver specific performance targets and project objectives, as agreed with the relevant line manager.

g) Work flexibly to meet the needs of the service users. This may, in exceptional circumstances, involve provision of telephone support hours outside of 9am to 5pm.

h) Develop and maintain good relations with other members of the project team, hospital teams, social work teams and community health providers (including pharmacists, AUKB teams, voluntary sector organisations and external practitioners).

i) Attend regular team meetings and appropriate training courses.

j) Maintain a working knowledge of community-based resources that are available to support people living with long term conditions.

k) Maintain records in line with all operational procedures and data protection legislation and produce written reports of work as required.

Person Specification

Qualifications

Essential

• A good level of numeracy and literacy.
• Must be willing to travel around North Somerset.

Desirable

• Professional qualification in a relevant area.

Experience

Essential

• Experience working with people with health and social needs, or community-based work.
• Experience building relationships with, colleagues, stakeholders, and other organisations.

Desirable

• Experience of using motivational Interviewing and strengths-based approach.
• Experience of using databases to record client actions and interventions.
• Experience of working in a hospital or community health setting.
