Job summary
West Hove PCN are looking for a dynamic and motivated Clinical pharmacist to join their PCN pharmacist team. The new team member will provide direct clinical support to West Hove Primary Care Network in relation to Medicines Optimisation and Management, Medication and Prescribing Issues and Management of Common and Long-Term Conditions providing high quality patient focused care for the patients of the PCN member practices.
The new post offers the successful candidate an opportunity to develop the role forging relationships between the existing practice pharmacists and being involved in the reconfiguration of medicine management within the network. There will be a balance of face to face patient contact and a wider brief of network support. The network will provide peer-peer support, mentoring and encourages career development.

Main duties of the job
The post holder is a pharmacist supporting and working alongside a team of pharmacists in general practice.
The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries.
They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s).
The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
The post holder will be supported to develop their role to become a nonmedical prescriber.

West Hove PCN consists of 6 practices with a population size of 49,452 patients. The six constituent practices are; Portslade Health Centre, Wish Park Surgery, Benfield Valley Health Care Hub, Mile Oak Medical Centre, Links Road Surgery and Hove Medical Centre
