Job summary
Working up to 37.5 hours a week, you
will lead, support and supervise our team of Health Coaches and Social
Prescribing Link Workers to provide personalised support to individuals,
their families and carers to take control of their health and wellbeing; live
independently and improve their health outcomes. To take a holistic approach to
addressing non-medical/social issues in their life, such as housing, debt or
social isolation by introducing or connecting people to community groups,
activities or statutory services.
You will also manage a caseload of primary care patients using
evidence based and patient centred principles to assess, plan, implement and
evaluate interventions. This is a developing role with the primary care network and we expect the post holder(s) to contribute to the development of the role and community resources locally.
Main duties of the job
We are looking for a motivated individual who is able to
work independently to manage their own workload, and are flexible to respond to
the developing nature of the role and the needs and priorities of individuals
requiring support. You will be expected to work collaboratively with a wide
range of partner organisations, professionals and volunteers to improve the
health and wellbeing of Copeland patient population.
You will hold
BSc Occupational Therapy under an approved
training programme, be a registered member of Health and Care
Professions Council (HCPC). You will also need evidence of ongoing professional development, min 2 years experience as practicing OT & experience of supervising and/or line management
of a staff team
About us
Copeland Primary Care Network, is a network of 7 practices covering the Copeland district health population from Distington to Bootle.
You will join a developing service within the network; The Community Wellness Service, which is designed to provide support to people to improve their health and wellbeing. The team is made up of health coaches and social prescribers, supported by the wider primary care team and specialist roles like physiotherapist and learning disability nurse.

Main duties &
responsibilities
Take line
management responsibility for the Community Wellness team members including
social prescribers and health coaches; via formal and informal supervision
arrangements including annual appraisals, peer support and team meetings.
Lead on the
development of the service and the team; to include recruitment and selection
of new staff members
Manage and
co-ordinate the implementation of individual treatment plans for referred
patients ensuring treatments, activities and interventions are delivered
through effective application and communication.
Receive and
respond promptly to referrals undertaking patient assessments to identify and
prioritise patient needs before planning and delivering the appropriate and
relevant interventions either on an individual or group basis. Maintains
accurate and timely records detailing each patients rehabilitation / progress.
Work with patients through a shared-decision making
approach to plan realistic, outcomes-focused goals.
Undertake both verbal and non-verbal communication
methods to address the needs of patients that have communication difficulties.
Involve MDT colleagues, physiotherapists, social
workers, alongside patients' families, teachers, carers and employers in
treatment planning, to aid rehabilitation.
Where appropriate, support the development of
discharge and contingency plans with relevant professionals to arrange on-going
care in residential, care home, hospital, and community settings.
Periodically review, evaluate, and change
rehabilitation programmes to rebuild lost skills and restore confidence.
Where appropriate, advise on home, school, and
workplace environmental alterations, such as adjustments for wheelchair access,
technological needs, and ergonomic support.
Teach coping strategies and support adaptation to
manage long term conditions for physical and mental health.
Advise on specialist equipment and organisations to
help with daily activities.
Maintain
and develop close working relationships with professional and clinical
colleagues within the multi-disciplinary team, ensuring clinical practice is in
line with policies and procedures and complies with statutory regulations and
quality standards.
- Support people to recognise and change their
current lifestyle and to identify how their way of life might affect their
health and well-being.
- Seek advice from the patients or lead GP as
appropriate.
- Ensure patients and their families/carers
views are taken into account in every stage of the decision making process.
