Job summary
Pike View - Dr Malhotra & Partners is a GP surgery located in the new Horwich Health & Wellbeing Hub with just over 9,000 patients.
The right candidate will help us to continue delivering safe and co-ordinated care to our patients with focus on chronic disease management, cytology, immunisations, contraception (pill/depo)
We boast a friendly and welcoming team, supportive clinical staff, NHS pension, 5 weeks annual leave plus bank holidays (pro-rata)
Main duties of the job
Implement and evaluate individual treatment plans for chronic disease patients
Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
Support the team in dealing with clinical emergencies
Implement vaccination programmes for adults and children
Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
Support the clinical team with all safeguarding matters, in accordance with local and national policies
Deliver opportunistic health promotion where appropriate

Pike View - Dr Malhotra & Partners is one of 3 practices that make up Horwich Primary care network (PCN) We have a small branch surgery located in Westhoughton.
We are a long standing high achieving practice in QOF & BQC
Our clinical system is EMIS alongside Docman 10, Ardens, Accurx, Iplato.
Due to a recent move to a brand new purpose built A+ energy rated building we are expanding our clinical and administrative team.

Job responsibilities include but not limited to:
- Develop, implement and embed health promotion and well-being programmes
- Implement and evaluate individual treatment plans for chronic disease patients
- Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
- Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Provide travel medicine services
- Request pathology services as necessary
- Process pathology results as required
- Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure read codes are used effectively
- Maintain chronic disease registers
- Chaperone patients where necessary Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Implement vaccination programmes for adults and children
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
- Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
