Job summary

We are recruiting for up to 6 new members of this team and we can offer flexible working either part time or full time where this fits with the current team planning. This post would suit either someone with previous clinical experience or without as full training will be given. The successful candidates will need to have their own transport and be able to work across a number of geographical locations including:

Edlesborough and Pitstone,Whitchurch, Wing,Waddesdon,Winslow,Buckingham and the surrounding villages included in the area.

After a period of induction, workers will be allocated to a practice which will become their principal base, unless required to cover for team colleagues annual/sick leave or fluctuations in workloads.

The Patient Support Service work within the Primary Care Network of Practices in the North of Buckinghamshire. The service offers patients a home-based holistic assessment, to help with a wide range of health, social and mental health issues.

Main duties of the job

Aim of the post:

To work as a member of the Patient Support Service (PSS) team

To proactively find patients who might benefit from an assessment

To enhance the PSS profile within the surgery and local community

To liaise with GPs regarding patients who require additional care

To liaise with other members of the Multidisciplinary Team (MDT)

To act as MDT representative for the allocated surgery

To attend surgery meetings and feedback information regarding patients

To ensure colleagues are made aware of any service changes, offering guidance and help as required

About us

The Patient Support Service has been in place for 5 years and is growing in size as we try to support our patients to live healthy and independent lives.

The Primary Care Network consists of 5 practices over 10 sites who have a long history of working together to enhance the care offering in the locality.

The team support these patients to remain as independent as possible in their own home by accessing care, benefits, mobility aids,advice and in many other areas that they may need help. The team also seeks to combat loneliness and isolation by supporting the work of the volunteer Telephone Befrienders and signposting patients to appropriate, as well as relevant, social opportunities.
