Clinical practice:

The Advanced Clinical Practitioner (ACP) will work as an autonomous professional in a self-directed manner, with confidence, authority and expertise, undertaking comprehensive range of clinical assessments/procedures within Hollyns Health and Wellbeing. They will have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporate the issues and recommendations made.

They will assess individuals, families and populations holistically using a range of different assessment methods, using health promotion and prevention strategies; and comprehensively assess patients for risk factors and early signs of illness. Care will take place either the clinic setting or at the patients usual abode.

The Advanced Clinical Practitioner will be expected to interpret and analyse highly complex clinical and non-clinical facts to form accurate diagnoses, prognoses for a wide range of conditions, to recommend / deliver the best course of intervention including the development of comprehensive packages of care.

The Advanced Clinical Practitioner will plan, manage and complete episodes of care, working in partnership with others and delegating /referring as appropriate; to optimise health outcomes and resource use, as well as providing direct support to patients and clients. They will use professional judgement in managing complex and unpredictable care events and capture the learning from these experiences to improve patient care and service delivery.

The Advanced Clinical Practitioner will always maintain a high standard of patient focused care in accordance with relevant guidelines and policies. They will actively involve service users in providing feedback of their experience of the current service and suggestions for improvements.

Exemplary customer service and patient satisfaction is crucial and the post holder will ensure the very highest standards of care are provided for our patients, providing an environment where they are engaged with, cared for, and their privacy and dignity maintained at all times.

Leadership and Collaborative Practice:

The post holder will be able to implement systems to promote their contribution and demonstrate the impact of advanced level practice to the healthcare team and the wider health and social care sector.

The Advanced Clinical Practitioner will need to be resilient and determined and demonstrate leadership in contexts that are unfamiliar, complex and unpredictable. They will need to engage stakeholders and use high-level negotiating and influencing skills to develop and improve practice. They will also need to work across professional, organisational and system boundaries and proactively develop and sustain new partnerships and networks to influence and improve health, outcomes and healthcare delivery systems.

Improving Quality and Developing Practice:

The post holder will work within the NMC/HCPC Code of Professional Conduct and Guidelines, Hollyns Health and Wellbeing policies and guidelines, COSHH, the Mental Health Act and other instruments of statutory legislation to ensure safe and effective professional working practices for self and others.

Qualifications

Essential

• SKILLS, QUALIFICATIONS and other REQUIREMENTS
• Registered with relevant professional body
• Degree-level registration qualification or equivalent post- registration award
• Masters Degree in Advanced Practice is preferred
• Higher Level degree plus PG Diploma in Advanced Practice is advised
• Life Support qualifications (or alternative profession specific clinically determined qualifications)
• Relevant teaching qualification
• Prescribing (medical and non-medical) qualifications should be achieved within a reasonable time period

Experience

Essential

• Evidence of continuous professional development and the practical application of new skills to the work place.
• Evidence of significant practical contributions to the development of the specific service within the past 2- years.
• Supervision of students
• Evidence of lead for or contribution to audit programmes within practice
• Change Management experience and direct involvement of redesign within a service, including promoting evidence based practice.
