Job summary

Hollyns Health and Wellbeing is seeking a part-time or full time experienced and enthusiastic Advanced Clinical Practitioner from any appropriate AHP background with MSc in Advanced Practice.

An Independent prescriber qualification is desirable but not essential.

GP Practice experience is desirable.

The Surgery has

  • 13,800 patients approx.

  • GPs, Nurses, Health Care Assistants, Pharmacists, Advanced Clinical Practitioners, Advanced Nurse Practitioners, Physician Associate, Administration staff

  • Chronic disease clinics for diabetes, asthma and COPD

  • Rated Good by CQC

  • Active GP and Nursing training and teaching practice

  • SystmOne clinical system

  • High QOF achievement

  • Part of a Primary Care Network with access to additional pharmacists, social prescribers, physiotherapy

  • Minor surgery, sexual health, diabetes, anticoagulation and pain management services available for patients

Main duties of the job

  • Varied role involving telephone, video and face to face consultations, telephone triage, palliative care management, home visits

  • Ability to autonomously provide clinical assessment, diagnosis and treatment of patients presenting with undifferentiated, undiagnosed conditions as well as those with chronic diseases

  • Aware of the latest Government guidelines working in Primary Care in relation to Covid 19

  • Independent prescriber

  • GP Practice experience desirable

  • Working days Monday to Friday

  • Salary negotiable dependent on experience

About us

Hollyns Health and Wellbeing is a proactive practice with two sites, who offer additional services to the practice population, such as warfarin, pain management, sexual health, Level 2 diabetic clinics and minor surgery.

The administration, nursing and clinical team work over both sites and teamwork is integral in ensuring the service is run as effectively and efficiently for the patients as possible.

The successful candidate will be able to continue their service benefits if coming from the NHS in relation to the NHS Pension Scheme and annual leave entitlement.

Clinical practice:

The Advanced Clinical Practitioner (ACP) will work as an autonomous professional in a self-directed manner, with confidence, authority and expertise, undertaking comprehensive range of clinical assessments/procedures within Hollyns Health and Wellbeing. They will have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporate the issues and recommendations made.

They will assess individuals, families and populations holistically using a range of different assessment methods, using health promotion and prevention strategies; and comprehensively assess patients for risk factors and early signs of illness. Care will take place either the clinic setting or at the patients usual abode.

The Advanced Clinical Practitioner will be expected to interpret and analyse highly complex clinical and non-clinical facts to form accurate diagnoses, prognoses for a wide range of conditions, to recommend / deliver the best course of intervention including the development of comprehensive packages of care.

The Advanced Clinical Practitioner will plan, manage and complete episodes of care, working in partnership with others and delegating /referring as appropriate; to optimise health outcomes and resource use, as well as providing direct support to patients and clients. They will use professional judgement in managing complex and unpredictable care events and capture the learning from these experiences to improve patient care and service delivery.

The Advanced Clinical Practitioner will always maintain a high standard of patient focused care in accordance with relevant guidelines and policies. They will actively involve service users in providing feedback of their experience of the current service and suggestions for improvements.

Exemplary customer service and patient satisfaction is crucial and the post holder will ensure the very highest standards of care are provided for our patients, providing an environment where they are engaged with, cared for, and their privacy and dignity maintained at all times.
