Job summary
To work as an autonomous practitioner working as part of the practice multidisciplinary team, delivering care within their scope of practice to the eligible patient population. The advanced nurse practitioner will hold a key role in managing acute minor illness to support GP colleagues. The ANP will provide care for patients presenting at the practice from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within general practice.
We are looking for a confident practitioner ready to face the challenges of primary care within a supportive and friendly team.
Part-time/Job share would be considered.
Main duties of the job
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
Work with patients in order to support compliance with and adherence to prescribed treatments.
To undertake care home ward rounds.
An interest in Diabetes Care would be desirable but not essential.

We are a 7 GP training practice, looking after 10,000 patients. Our Nursing Team consists of 3 Practice Nurses, 2 Healthcare Assistants and 1 Nurse Associate. We are located on the border of Dudley, Romsley and Birmingham. We look after 2 care homes including an Advanced Dementia Unit. We are the lead practice for Halesowen Primary Care Network and our Senior Partner, Dr Gillian Love is the Clinical Director.
We have an established team of Primary Care Network staff to support the network allowing us to offer a variety of services to include Health and Well-being, Dementia and Frailty Support, First Contact Physiotherapy, Nurse Associate Services, Phlebotomy and Listening and Guidance Services.
We use Emis Web, Docman and Footfall.
