We are a very friendly and supportive team, co-located in a purpose built Health Centre with a wide range of associated health services.

About us

We are pleased to be able to offer flexibility in working hours and days and an informal chat prior to interview would be welcomed. This exciting opportunity to join our friendly and dynamic team will include a balanced mix of on call/urgent same day and routine general practice work including home visits, reviewing results and reviewing patient documents using Docman. We have recently completed programmes to improve efficiencies within the practice to enable more time for patient care and team catch-ups. We have regular practice meetings and regular CPD sessions in-house.

Job Summary:

The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. The post holder will demonstrate critical thinking in the clinical decision making process. They will work collaboratively with the general practice team to meet the needs of patients.

The post holder will support the Practice in continually reviewing how we deliver our nursing services to meet increasing patient demand, recommending and implementing change to ensure we are delivering a modern, efficient and coordinated service.

Clinical Responsibilities:

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  • Be involved in actively recruiting patients to research studies
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Monitor and develop individual management plans; agreeing these as appropriate with the patient, carers and other health professionals
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  • Provide telephone assessment to patients and invite them to attend the surgery if appropriate
  • Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
