opportunity has arisen to join our multi-disciplinary PCN Team due to the
changes in the ARRS Scheme, we are looking to expand our team by seeking to
employ a newly qualified General Practitioner to work alongside our practices
and PCN staff.
Our
PCN staff currently includes Clinical Pharmacists, Social Prescribers, Health
& Wellbeing Coach, Mental Health Practitioner, Paramedics, First Contact
Physiotherapist, GPAs, Pharmacist Technicians, PCN Manager/Digital &
Transformation Lead and a Care Home Team.
The
successful applicants will work alongside the PCN and practice teams to support
healthcare professionals, patients, and carers.

Main duties of the job
To perform as an
autonomous general practitioner working independently and with other health
care professionals to assess, diagnose and treat the conditions of patients
attending within a primary care setting. Provide expert professional advice to
patients, carers and colleagues and ensure the maintenance of clinical
excellence. To be involved in the provision of education and training to GP
Registrars and other staff.
To be professionally
accountable and responsible for all elements of clinical practice in accordance
with the GMC code of conduct ensuring the safety of patients and quality of
their care is primary.
The post holder will
need to be capable of independent travel across the Preston and, South Ribble area.
This role is a fixed term position for 12 months, 5 sessions per week to be agreed.

GP practices have been working together in our area for years and formed BridgedalePrimary Care Network (PCN).Bridegale Primary Care Network is made up of GP registered patient lists for Roslea Surgery, Riverside Medical Centre and the Ryan Medical Centre. We serve a community of around 30,000 people. We are small enough to provide the personal care valued by our people and large enough to have impact and economies of scale through better collaboration.
As a Primary Care Network we are working hard to deliver on the national NHS England network contract that commenced in July 2019 and continues to March 2024.
This includes the following areas of focus:

cardiovascular disease prevention and diagnosis
enhancing health in care homes
early diagnosis of cancer
tackling neighbourhood inequalities
structured medication reviews
social prescribing reviews

In
accordance with the Practice/PCN rota, as agreed, the post-holder will make themself
available to undertake a variety of duties including surgery consultations,
telephone consultations and queries, visiting patients at home and in Care
homes, checking and signing/authorising repeat prescriptions and dealing with
queries, paperwork, and correspondence in a timely fashion.
o Making professional, autonomous
decisions in relation to presenting problems, whether self-referred or referred
from other health care workers within the organisation.
o Assessing the health care needs of
patients with undifferentiated and undiagnosed problems
o Screening patients for disease risk
factors and early signs of illness
o In consultation with patients and in
line with current practice disease management protocols, developing care plans
for health.
o Providing counselling and health
education
o Admitting or discharging patients to
and from the caseload and referring to other care providers as appropriate
o Recording clear and contemporaneous
consultation notes to agreed standards.
o Collecting data for audit purposes
o Compiling and issuing
computer-generated acute and repeat prescriptions
o Prescribing in accordance with the
practice prescribing formulary (or generically) whenever this is clinically appropriate.

visit patients/care homes during surgery time in an emergency if clinically indicated.

% of on call sessions per week as allocated on the PCN rota.
