Job summary

An Advanced Clinical Practitioner is an experienced clinician who demonstrates expertise in their scope of practice. Advanced Clinical Practitioners manage defined episodes of clinical care independently, from beginning to end. They provide care and treatment from the time an individual first presents through to the end of the episode, which may include admission, referral or discharge, or care at home. They carry out their full range of duties in relation to individuals physical and mental healthcare and in acute, primary, urgent, and emergency settings (including hospitals, general practice, individuals homes, schools and prisons, and in the public, independent, private and charity sectors). They combine expert clinical skills with research, education and clinical leadership within their scope of practice.

Advanced Clinical Practitioners work innovatively on a one-to-one basis with individuals as well as part of a wider team. They work as part of the wider health and social care team and across traditional professional boundaries in health and social care.

Main duties of the job

This is a two-year programme in collaboration with Manchester Metropolitan University; you will be working alongside an exceptional Clinical team who are very active in our PCN.

The course runs for two yearsMust have Level 2 Maths and English to applyTwo days learning required per week typically one day in university each WednesdayThere are two flexible choice units (opportunity for APL if people have relevant prior learning)

Core units always take place on Wednesday, and we suggest that two days per week are allocated to completing university work and corresponding supervision, clinical skills practice etc.

We are an innovative friendly practice with excellent staff retention among their existing clinical and non-clinical team. We have a broad Multi-Disciplinary Team via the PCN including First Contact Physios, Mental Health Nursing and Health & Wellbeing coaches/ Focused Care Workers

The practice offers a friendly atmosphere, with an open-door policy and regular meetings with the multi-disciplinary team.

  • Experience of telephone triage assessment and relevant pathways.
