Job summary

We have an exciting and innovative new opportunity for a Care Co-ordinator to work for Sphere Primary Care Network (PCN). The role involves working with a team of clinicians to enable people to live healthy independent lives and support active self-management and prevention. The candidate will possess excellent communication and organisation skills and have experience in a Health, Social Care or Educational background workplace.

The successful candidate will join our growing PCN workforce and support the delivery of care bringing together all the information about a patient's identified care and support needs and exploring options to meet these by identifying and signposting to appropriate clinicians.

Main duties of the job

We require a full time Care Coordinator to join our team to provide and maintain high standards of patient care as well as administrative tasks. The successful candidate will enjoy working as part of a dedicated team, with the added satisfaction of working for an organisation committed to the care and wellbeing of others.

Specific areas of work include supporting General Practices and the Primary Care Network to deliver the Local Enhanced Services including Improving Diabetes Care to our population living with Diabetes. You will also help with the development of Sphere Primary Care Network digital platform; help design, develop and manage the Sphere Primary Care Network Website.

Roles and Responsibilities:

1. Personalised care planning

Work with patients to understand their health and wellbeing needs. Help them make informed choices and create a clear care plan that brings together the right services to support them.

2. Supporting self care

Use simple tools to assess how confident patients feel about managing their health. Based on this, guide them towards education, peer support groups, or other resources to build their skills and independence.

3. Wellbeing support

Link people to opportunities for training, work, or benefits advice to improve their wider quality of life.

4. Helping with appointments and follow-ups

Answer patient questions, book appointments, and make sure follow-up checks (for conditions like diabetes, asthma, or kidney disease) happen on time.

5. Teamwork across the PCN

Share information and act as a key contact between different healthcare professionals. Help organise and run team meetings across the network.

6. Practice support and admin

Keep patient records and registers up to date. Support practices with admin tasks such as recalls, inbox management, and sorting out data issues, escalating concerns where needed.

7. Digital and communications

Help maintain the PCNs website, newsletters, and digital tools. Write and upload clear, accurate content.

8. Patient support

Be a first point of contact for patients by phone or email, offering clear and friendly help.

9. Using EMIS (clinical IT system)

Build and maintain reports and tools within EMIS to track appointments, patient data, and performance targets.

10. Referrals and clinic support

Handle referrals to clinics (e.g. for asthma, kidney disease, or childrens services) and provide technical support during specialist clinics when needed.

11. Enhanced Services

Support the monitoring and reporting of additional NHS services delivered by the PCN, making sure activity is recorded accurately and cost-effectively.

12. Policies and standards

Stay up to date with local and national service requirements, and make sure the PCNs work aligns with NHS standards.
