 Witterings Medical Centre is seeking an Advanced Clinical Practitioner (ACP) to be part of our multi-disciplinary and multi-agency team of professionals working across the Witterings and Birdham footprint. This is open to anyone with relevant registration. This is an exciting and unique opportunity to be at the forefront of the delivery of our new Ageing Well model of care. Working both independently, and within our multi agency team, the role will operate across the traditional health and social care organisational boundaries, including our GP practice partners, to deliver a holistic approach to acute on day / rapid response services.

We are looking for an Advanced Clinical Practitioner with a strong interest in working in a GP setting and who aspires to provide a continuous improvement approach to delivering responsive, safe, patient-centred and effective care within a team environment.

Main duties of the job

The service supports patients with multifaceted health problems living in their own homes as well as in care homes. The service liaises with care home staff, carers, families and colleagues to maintain and maximise individualised patient-centred care (including advanced care planning). The successful post holder will work autonomously within their professional boundaries to:

• Undertake comprehensive health assessments, initial history taking, clinical assessment, physical examination, diagnosis, treatment and evaluation of their care

• Use advanced clinical decision making and assessment skills, coordinate care planning and initiate onward referrals as appropriate

• Demonstrate sound safe clinical decision–making.

Job responsibilities

The Advanced Clinical Practitioner (ACP) will act autonomously within their professional scope of practice providing care for patients face to face, video/telephone consultations and home visits.

The post holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.

The ACP will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.

The post holder will be an experienced nurse who holds an appropriate, specific, advanced professional qualification with the ability to use skills, knowledge and competencies in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.

Job responsibilities:

  • Work as part of a multi-disciplinary team to deliver safe and high quality care.
  • Carries out clinical practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.

· Delivery of care in line with NSF, NICE guidelines and evidence-based care

  • Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.

· Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.

  • Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
  • Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.

· Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.

  • Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.
  • Refers patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.

Qualifications

Essential

  • Registered Adult Nurse registered with the Nursing and Midwifery Council (NMC) or,
  • Paramedic registered with the Health Care Professions Council (HCPC) or
  • Pharmacist registered with the General Pharmaceutical Council (GPhC).
  • Registered as an independent nonmedical prescriber.
  • A minimum of 2 modules at Level 7. One module must have included assessment of clinical examination skills

Desirable

  • MSc (Advanced Practice)
  • Mentorship qualification

Experience
