Job summary

Trowbridge PCN are pleased to announce an exciting vacancy for a Digital and Transformation Lead to support the two practices within Trowbridge Primary Care Network.

Are you forward thinking and have the ability to embrace digital improvement?

Do you have experience of administering general HR procedures?

Are you able to work with complex financial information for processing?

Main duties of the job

The successful candidate would work under the direction of the PCN board to administer strategy, implement change, work alongside key stakeholders, and identify potential strategies. As this role is a new, emerging role, NHS England have identified compulsory training for all new personnel who are joining this role.

About us

Trowbridge Primary Care Network consists of two GP Practices, covers the Trowbridge area and a population of almost 50,000. There is a strong team spirit throughout the organisation. Both practices enjoy working together to produce the most efficient services for our patients within our town.

Objectives of the role

The objective of the Digital and Transformation Lead is to provide a central source of leadership and management to support both practice managers with service transformation for our population.

The remit of this role is broad and will require attention to detail in helping to identify and tackle health inequalities and improve the quality of care and outcomes for our patients. Also, to deliver high-quality day-to-day operations.

Main Responsibilities

Strategic Planning

To lead and develop a strategic plan that includes a robust digital infrastructure

To clarify objectives and priorities for network delivery in line with operational capacity, financial parameters and contract requirements

Promote the engagement of both PCN practices and act as an ambassador for the network.

To proactively build relationships with system leaders to help influence proactive and new models of care

To use business intelligence tools to aid data-driven decisions

Operations

To provide oversight and accountability to the delivery of PCN contracts and specifications

Coordinate training and development of PCN staff

Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented.

To drive issues through to resolution, acting as a point of expertise

To implement network-based projects

HR and workforce Planning

Support recruitment, induction, training, performance reviews and elements of PCN staff management

Develop a coherent workforce plan to support recruitment.

Facilitate actions to promote PCN staff retention.

Finance and Governance

To support the collation of financial information.

To monitor and check the accuracy of the submissions for the DES and contractual requirements.

Compiling and presenting financial reports.

To ensure that the PCN's physical and financial resources are managed and maintained for maximum effectiveness.

Digital Infrastructure

To ensure the PCN maximises its business intelligence tools eg NHS App, Cloud based telephony, as per ICB guidelines and advice.

Develops a continuous improvement approach to digital accessibility for our patients and workforce eg NHS App, Online booking.

Embraces AI technology.

Creates data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff and patient experience.

Relationship Management:

To foster a culture of collaboration and promote learning to drive continuous improvement. As this role is new, the NHS require all DTLs to follow a course of learning to enable them to carry out their role.

To work as a flexible member of the PCN, providing support to other team members when necessary.

Develop and deliver a communication plan using a wide variety of channels and stakeholders.

To take an active role in the development and embedding of the PCN's culture, values and reputation.

To represent the network at external meetings.

Adhere to practice policies and procedures.

Adhere to data protection regulations.

Undertake a Disclosures and Barring Service application (DBS)

Be committed to safeguarding and promoting the welfare of children, young people and vulnerable adults.
