Job summary

The successful candidate will support the Clinical Pharmacists to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.

Main duties of the job

The postholder will support the Clinical Pharmacists providing medication reviews, adding and amending medication requests including the achievement of the practices prescribing on Quality and Outcomes Framework targets and the local Prescribing Incentive Scheme.

The postholder will respond to medication queries from patients and staff in a professional manner referring to the appropriate clinician in accordance with practice protocols.

The postholder will liaise where necessary with Community Pharmacists and other pharmacists in relation to medicine optimisation.

The postholder will take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.

The postholder will participate in medication and other clinical audits.

The postholder will support the Clinical Pharmacists in developing further Standard Operating Procedures, patient information leaflets and information about medicine awareness.

Rushall Medical Centre is a busy GP practice situated about 3 miles outside of Walsall Town Centre. The building is a large modern, purpose built practice. We pride ourselves on being a well-established training practice for trainee doctors and Pre-Reg Pharmacists. We are friendly, welcoming and a forward thinking practice with approximately 45 members of staff including GPs, Clinical Pharmacists, Practice Nurses, Healthcare Assistants, Administration Staff including ARRS staff which include Paramedics, First Contact Physiotherapist's, Social Prescribers, Health and Wellbeing Coaches, Bereavement and Cancer Support Care Coordinators. Our list size is approximately 15,000 and as a practice we believe in delivering the highest standard of care for all of our patients.

Job description

Job responsibilities

The postholder will work the Lead Clinical Pharmacist, Pharmacists and Clinicians in accordance with agreed practice prescribing standard operating procedures.

The postholder will process repeat medication requests, including the conversion of acute requests to repeat medicines and changing quantities of medicines.

The postholder will issue prescriptions in line with clinical protocols and within their competency

The postholder will provide advice and guidance to patients regarding medication optimisation and lifestyle, for example asthma reviews and diabetic reviews

To provide medication review services to patients and deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care

The postholder will support the achievement of the Quality and Outcomes Framework targets and the local Prescribing Incentive Scheme

The postholder will deal with medication queries including acute medication requests, patient medication and prescription queries

The postholder will be expected to update and maintain accurate patient medication records on the practice clinical system including advice given and action taken

The postholder will liaise with secondary and primary care colleagues to ensure correct medicines are continued following the transfer of care

The postholder will assist the Lead Clinical Pharmacist with data analysis of prescribing behaviour in line with NICE guidance and local clinical commissioning directives

The postholder will assist with the development and review of medicine audits

The postholder will be a contact for patients and colleagues with medication queries

The postholder will do specific work/prescribing initiatives under the support of the Lead Clinical Pharmacist

The postholder will conduct medication reconciliation following discharge from hospital, clinic visits and other transfers of care (eg new patients, care home residents) to ensure that medication changes are safely implemented in a timely fashion

The postholder will provide a link with hospital and community pharmacy colleagues to address discrepancies, supply issues and queries within their limits of competence

The postholder will liaise with the wider healthcare team including primary care staff, community nurses and care home staff with medication queries

The postholder will undertake regular audits using different tools, for example to identify priority patients for medication review

The postholder will support the safe and effective repeat prescribing of high-risk medicines ensuring that regular monitoring as per local/shared care agreements

The postholder will support patients to manage their medications, e.g. synchronisation of medications, reviewing patient's suitability for monitored dosage systems, and setting up electronic repeat prescribing

The postholder will document interventions made to patients medications in their healthcare records to ensure there is a clear audit trail and changes are clearly accounted for

The postholder will promote cost effective, safe, evidence-based prescribing in accordance with local formulary, medication optimisation strategy and national guidance

The postholder will seek advice from the patient's GP or Lead GP as appropriate

The postholder will link/liaise with local strategic groups to ensure work is aligned with local health and wellbeing priorities

The postholder will establish positive working relationships and effective communication with network practices, integrated care health and care teams, and a range of local voluntary sector and community organisations

The postholder will utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment

The postholder will use highly developed knowledge and skills to provide high standards of patient centred care

The postholder will maintain accurate and contemporaneous records on all aspects of the care process and patient contact

The postholder will contribute to the development of the service e.g. developing service pathways, paperwork and sharing good practice across the network
